Background: While Epidermal growth factor receptor (EGFR) mutation-positive lung adenocarcinoma (LUAD) has favorable outcomes with targeted therapy, early-stage prognosis remains influenced by pathological factors and central nervous system (CNS) recurrence. The study aimed to clarify prognostic factors in pathological stage (pStage) I EGFR mutation-positive LUAD.

Methods: Between 2015 and 2018, 2,191 pStage I LUAD cases with known EGFR status (excluding EGFR testing after recurrence) who received anatomical resection were included from multiple institutions in Japan.

Background: With advancements in minimally invasive thoracic surgery techniques, such as video-assisted thoracoscopic surgery and robotic surgery, the design of vascular staplers has evolved to meet the requirements of these procedures. Consequently, newer generations of automatic staplers with improved handling and reduced size have been introduced, such as two-row staplers, which are more maneuverable and less bulky than their three-row counterparts.

Case Presentation: A 68-year-old man with lung cancer underwent a right middle and lower lobectomy due to tumor invasion into the central middle bronchial trunk, rendering the preservation of the middle lobe impossible.

Objective: Focal cortical seizures travel long distances from the onset zone, but the long-distance propagation pathways are uncertain. In vitro and in vivo imaging techniques have investigated the local spread of seizures but did not elucidate long-distance spread. Furthermore, classical studies in slices suggested seizure spread locally along deep cortical layers, whereas more recent in vivo imaging studies posit a role for superficial cortical layers in local spread.

Background: Mature teratomas are benign cystic tumors that are most commonly asymptomatic. However, in some cases, mediastinal teratomas rupture the lungs and mediastinum with potentially fatal outcomes. Herein, we report a case of a large mediastinal mature teratoma that expanded to the entire left hemithorax in a child with common cold-like symptoms.

Article Synopsis
  • - The study examined the role of programmed cell death-ligand 1 (PD-L1) expression and epidermal growth factor receptor (EGFR) mutations as biomarkers for lung adenocarcinoma, analyzing data from 847 patients across 21 centers in Japan from 2015 to 2018.
  • - Results showed that while PD-L1 expression was linked to shorter recurrence-free survival (RFS) in both EGFR-mutant and wild-type patients, it had no effect on overall survival (OS) in EGFR-mutant patients.
  • - The findings suggest that PD-L1 expression is a poor predictor of OS for EGFR-mutant patients and does not influence the effectiveness of post-relapse treatment with EGFR-

In patients with non-small cell lung cancer (NSCLC) who present with radiologically undetermined malignant pleural dissemination or incidental surgical diagnosis of the same, surgery is generally not the preferred option; systemic therapy is favoured. However, there is no consensus on incorporating primary site resection into the treatment plan. Retrospective analyses hint at potential benefits of combining systemic therapy with primary site resection, but prospective studies have yet to confirm these findings.

Objectives: The clinical trial showed that sublobar resection was not inferior to lobectomy in terms of disease-free survival in patients with peripherally located non-small-cell lung cancer ≤2 cm. However, it is not clear whether sublobar resection is indicated for all types of c-stage IA lung cancer. The purpose of this study was to clarify whether sublobar resection is indicated for c-stage IA hypermetabolic lung cancer.

This single-arm multi-institutional prospective study aimed to evaluate the 10-year outcomes of sublobar resection for small-sized ground-glass opacity-dominant lung cancer. Among 73 patients prospectively enrolled from 13 institutions between November 2006 and April 2012, 53 ground-glass opacity-dominant lung cancer patients underwent sublobar resection with wedge resection as the first choice. The inclusion criteria were maximum tumor size of 8-20 mm; ≥ 80% ground-glass opacity ratio on high-resolution computed tomography; lower F-fluorodeoxyglucose accumulation than the mediastinum; intraoperative pathological diagnosis of adenocarcinoma in situ; and no cancer cells on intraoperative cut margins.

  • Scientists studied how sleep is regulated in male mice, especially after staying awake for a long time.
  • They found that a special protein called CaMKII helps control certain brain cells (called PV neurons) that are important for sleep.
  • When these PV neurons become more active, it leads to more rebound sleep, which happens after being awake for too long.

Article Synopsis
  • The study compares survival rates between two surgical procedures—segmentectomy and wedge resection—for patients with early-stage non-small cell lung cancer (cT1cN0M0).
  • A total of 91 patients were analyzed, showing no significant difference in overall survival after 5 years (67.7% for segmentectomy vs 52.0% for wedge resection), but a notably worse recurrence-free survival for the wedge resection group (46.9% vs 66.6%).
  • The findings suggest that while both surgeries have similar overall survival rates, segmentectomy may offer a better prognosis in terms of recurrence-free survival.

  • The focus of postoperative monitoring for patients with non-small cell lung cancer is to identify any recurrence or new cancers while treatments are still effective.
  • Although guidelines suggest regular CT scans, evidence shows they may not actually improve patient survival, particularly in advanced stages of the disease.
  • A new phase III trial starting in October 2022 aims to test the effectiveness of less frequent CT scans compared to standard monitoring, involving 1100 patients across 45 institutions over four years, with overall survival as the main goal.

Background: Benign mature teratomas are the most common type of anterior mediastinal germ cell tumor. Mature intrapericardial teratomas are generally diagnosed during infancy because of symptoms of cardiac compression. In contrast, mature adult intrapericardial teratomas are extremely rare, accounting for less than 1% of mature intrapericardial teratomas.

Article Synopsis
  • - The study analyzed the effects of central nervous system (CNS) metastasis on performance status, treatment responses, and survival rates in lung adenocarcinoma patients with common EGFR mutations following surgical resection.
  • - Out of 4181 patients, 78 experienced CNS metastasis, which was more common in those receiving conventional adjuvant chemotherapy; however, this treatment showed no significant benefits in relapse-free survival or overall survival post-relapse.
  • - The findings suggest that CNS metastasis doesn't noticeably impact performance status or alter treatment efficacy with EGFR-tyrosine kinase inhibitors, indicating a need for more studies on managing CNS metastasis in the context of adjuvant TKI therapies.

Article Synopsis
  • The study investigates whether EGFR mutations influence postoperative recurrence in lung adenocarcinoma, finding that while a significant percentage (48%) of patients had these mutations, they did not independently affect survival outcomes.
  • A total of 840 patients who underwent surgery from 2005 to 2012 were analyzed, with follow-up showing similar five-year recurrence-free and overall survival rates between those with EGFR mutations and those without.
  • Ultimately, the research concludes that EGFR mutation status is not a reliable risk factor for recurrence or a predictor of survival in completely resected lung adenocarcinoma.
