A 6-year-old boy with an isolated fracture of the capitate was presented. First radiograghs at our clinic revealed an oblique fracture with a displacement of 5 mm on the lateral view, which we successfully treated with immobilization. When complete union was recognized in the radiographs, the capitate had a deformity consisting of a large prominence on the palmar aspect.

Soft tissue injuries such as damage to the cartilage and ligaments, soft tissue impingement, and the existence of free bodies within the intra-articular space account for this poor outcome. In treating fresh malleolar fractures, an arthroscope is used at our institution to confirm anatomic reduction on the articular surface and treat intra-articular injuries.

The case of a ganglion that had developed around the first metatarsophalangeal (MTP) joint and in which several other therapeutic approaches were unsuccessful was treated using an arthroscopic assisted procedure. During MTP arthroscopy, we found that the pedicle of the tumor was attached to the interior of the joint, and the lesion was successfully extirpated. Communication with the cyst was found on the medial side of the lateral sesamoid bone, which coincided with the rounded portion of the first MTP joint at the sole, where stress is exerted repeatedly while walking.

We describe a new therapeutic procedure using a bone filler in a calcium phosphate paste (bone paste) for endoscopic surgery for delayed unions and nonunions. Guided by radiography, the site of nonunion was confirmed. Two portals were created toward the nonunion site and, through each portal, the tissue around the nonunion site was released.

Bony reconstruction was achieved in 57 of the 60 cases; however, various postoperative complications occurred in 54 percent of the cases. One case of arterial thrombosis of an anastomosed vessel and nine cases of venous congestion of the monitoring flap occurred in the early postoperative periods.
