Sexual and gender minorities (SGMs) experience critical barriers to health care access and have unique health care needs that are often overlooked. Given the rise in individuals identifying as lesbian, gay, bisexual, transgender, and queer, colorectal surgeons are likely to care for increasing numbers of such individuals. Here, we discuss key barriers to health care access and research among SGM populations and outline approaches to address these barriers in clinical practice.

Background: To meaningfully understand outcomes of gender-affirming care, patient-reported outcome measures (PROMs) that are grounded in what matters to individuals seeking care are urgently needed. We present a case of an adolescent who sustained multiple facial fractures following a high-speed, head-on skiing collision. The patient presented to a level 1 trauma centre with bilateral LeFort I, LeFort II, naso-orbital ethmoid fractures and superior maxillary displacement to the level of the orbits requiring a staged operative approach to (1) disimpact the maxillary LeFort I segment and (2) reduce and fixate the multilevel facial fractures. The patient was discharged home with close follow-up, and after 1 year, had preservation of appropriate facial proportions without complications.

For gender-diverse individuals assigned male at birth, postpubertal estrogen and testosterone-lowering therapies alone are often inadequate to reverse the testosterone-related facial characteristics projecting a male sex identity. Facial feminization surgery significantly improves quality-of-life outcomes for gender-diverse patients by relieving this gender incongruence of the face. Because the shape of the forehead is so radically different between men and women, feminization of this area is critical for many individuals.
