Background: The Pooled Cohort Equations (PCEs) do not accurately estimate atherosclerotic cardiovascular disease (ASCVD) risk in certain populations. The 2018 AHA/ACC cholesterol guideline identified risk-enhancing factors as a supplement to PCEs-based risk assessment. However, the role of each risk-enhancing factor in ASCVD risk assessment has not been well quantified.

Background: Few tools exist for the early identification of patients at risk for chronic lung allograft dysfunction (CLAD). We previously showed hyaluronan (HA), a matrix molecule that regulates lung inflammation and fibrosis, accumulates in bronchoalveolar lavage fluid (BALF) and blood in CLAD. We aimed to determine if early posttransplant HA elevations inform CLAD risk.

Individual animals differ in their traits and preferences, which shape their social interactions, survival, and susceptibility to disease, including addiction. Nicotine use is highly heterogenous and has been linked to the expression of personality traits. Although these relationships are well documented, we have limited understanding of the neurophysiological mechanisms that give rise to distinct behavioral profiles and their connection to nicotine susceptibility.

Article Synopsis
  • The text discusses the challenges and potential of using organs from donors who die after cardiac arrest (DCD), highlighting that while there’s a significant need for donor organs, the outcomes for these organs have been historically poor.
  • Normothermic regional perfusion (NRP) is introduced as a technique that can enhance the outcomes and increase the use of DCD organs, but there is inconsistency in how different US organ procurement organizations (OPOs) implement it.
  • A survey of 55 OPOs revealed variations in NRP practices, with 49 OPOs having conducted NRP cases; most respondents expressed a need for standardized guidelines to improve the consistency and effectiveness of NRP procedures.

Increasingly, interventional thoracic workflows utilize cone-beam CT (CBCT) to improve navigational and diagnostic yield. Here, we investigate the feasibility of implementing free-breathing 4D respiratory CBCT for motion mitigated imaging in patients unable to perform a breath-hold or without suspending mechanical ventilation during thoracic interventions. Circular 4D respiratory CBCT imaging trajectories were implemented on a clinical robotic CBCT system using additional real-time control hardware.

RNF168 orchestrates a ubiquitin-dependent DNA damage response to regulate the recruitment of repair factors, such as 53BP1 to DNA double-strand breaks (DSBs). In addition to its canonical functions in DSB signaling, RNF168 may facilitate DNA replication fork progression. However, the precise role of RNF168 in DNA replication remains unclear.

Newborn genomic sequencing (NBSeq) has the potential to substantially improve early detection of rare genetic conditions, allowing for pre-symptomatic treatment to optimize outcomes. Expanding conceptions of the clinical utility of NBSeq include earlier access to behavioral early intervention to support the acquisition of core motor, cognitive, communication, and adaptive skills during critical windows in early development. However, important questions remain about equitable access to early intervention programs for the growing number of infants identified with a genetic condition via NBSeq.

The reaction of sulfur trioxide (SO) and thiobenzoic acid (CHCOSH) is investigated in the gas phase under supersonic jet conditions. Rotational spectroscopy of the parent and several isotopically substituted derivatives, in conjunction with DFT calculations at the M06-2X/6-311++G(3df,3pd) level of theory, identify the product as thiobenzoic sulfuric anhydride, CHC(=S)OSOOH. Single point CCSD(T)/CBS(D-T)//M06-2X/6-311++G(3df,3pd) calculations place the electronic energy of the product anhydride 114 kJ/mol lower than that of SO+CHCOSH at infinite separation.

Article Synopsis
  • Major depressive disorder in older adults (late-life depression) often leads to cognitive impairment, particularly in executive function, which may contribute to treatment resistance.
  • This study analyzed baseline cognitive data from 369 older participants in a clinical trial to understand the relationship between cognitive deficits and the effectiveness of pharmacotherapy for treatment-resistant late-life depression.
  • The findings revealed that participants exhibited significant challenges in inhibitory control and processing speed, with deficits in set shifting specifically predicting poorer response to treatment, indicating a need for tailored therapeutic approaches.

Background: HIV treatment programs in Africa have implemented centralized testing for routine viral load monitoring (VLM), which may result in specimen processing delays inhibiting timely return of viral load results. Decentralized, point-of-care (PoC) VLM is a promising tool for expediting HIV clinical decision-making but remains unavailable in most African settings. We qualitatively explored the perceived feasibility and appropriateness of PoC VLM to address gaps along the viral load monitoring continuum in rural Uganda.

The maternal regulation of diapause is one type of phenotypic plasticity where the experience of the mother leads to changes in the phenotype of her offspring that impact how well-suited they will be to their future environment. females with a diapause history produce offspring that cannot enter diapause even if they are reared in a diapause inducing environment. Accumulating evidence suggests that microRNAs regulate diapause and, possibly, maternal regulation of diapause.

Malaria has been a deadly enemy of mankind throughout history, affecting over 200 million people annually, along with approximately half a million deaths. Resistance to current therapies is of great concern, and there is a dire need for novel and well-tolerated antimalarials that operate by clinically unexploited mechanisms. We have previously reported that both tambjamines and prodiginines are highly potent novel antiplasmodial agents, but they required rigor optimizations to enhance the oral efficacy, safety, and physicochemical properties.

How nicotine acts on developing neurocircuitry in adolescence to promote later addiction vulnerability remains largely unknown, but may hold the key for informing more effective intervention efforts. We found transient nicotine exposure in early adolescent (PND 21-28) male mice was sufficient to produce a marked vulnerability to nicotine in adulthood (PND 60 + ), associated with disrupted functional connectivity in dopaminergic circuits. These mice showed persistent adolescent-like behavioral and physiological responses to nicotine, suggesting that nicotine exposure in adolescence prolongs an immature, imbalanced state in the function of these circuits.

Objective: Hindbrain herniation (HH) is a clinical prerequisite for prenatal repair of myelomeningocele/myeloschisis; however, a subset of patients lack HH on initial fetal imaging and may ultimately progress to exhibit herniation on subsequent prenatal or postnatal imaging. The authors sought to explore the cohort of patients without HH at the time of initial fetal consultation for myelomeningocele/myeloschisis repair to define their clinical characteristics and outcome.

Methods: From July 2016 to July 2022, patients evaluated at the Children's Hospital of Philadelphia Center for Fetal Diagnosis and Treatment for myelomeningocele/myeloschisis were classified into two cohorts: those with and those without HH.
