Introduction: Traumatic injuries can affect respiration both directly and indirectly. This study aimed to evaluate the predictive factors of need for mechanical ventilation (MV) following traumatic injuries.

Methods: This Retrospective registry-based cross-sectional study comprised trauma patients admitted to a major referral trauma center in Iran, from March 28, 2019, to January 31, 2023, identified within the National Trauma Registry of Iran (NTRI).

Objective: Road traffic injury (RTI) is the second leading cause of death and disability, and motorcycle crashes rank as the first cause of traffic fatality in Iran. We aimed to compare various characteristics between traumatized motorcycle riders and pillion passengers registered with the National Trauma Registry of Iran (NTRI).

Methods: This is a retrospective study of the NTRI, an ongoing multicenter trauma database built on a registry.

Background: Fibrous dysplasia is a benign fibro-osseous lesion where normal bone is replaced with immature dysplastic woven bone and fibrous tissue. Fibrous dysplasia has the potential to involve multiple bones of the craniofacial area in a rare condition. Management of this involvement type should be assessed carefully.

Background: T follicular helper (Tfh) cells are a subdivision of T helper cells involved in antigen-specific B cell immunity. Tfh cells play an essential role in the interaction of T cells/B cells in the germinal centers (GC), and dysregulation of Tfh actions can offer pathogenic autoantibody formation and lead to the development of autoimmune diseases. This study seeks to evaluate changes in Tfh frequency and its related cytokines in autoimmune disease, its association with disease phase, severity, prognosis, and the effect of immunosuppressive treatment on the Tfh population.

Article Synopsis
  • Abdominal injuries are a major global health issue, often categorized into blunt and penetrating types, and this study focuses on their epidemiology and clinical characteristics using data from Iran's National Trauma Registry from 2016 to 2023.
  • Out of 532 patients studied, the majority (78.9%) had blunt injuries, predominantly affecting the spleen and liver, while penetrating injuries primarily involved the colon and small intestine.
  • Key findings include higher rates of ICU admissions and blood transfusions in blunt trauma cases compared to penetrating injuries, with significant predictors for ICU admission being Injury Severity Score (ISS) and the type of management (NOM vs. OM).

Background: Lengthy hospitalization may lead to an increased hospital-acquired patient complication, including infections, as well as increased costs for both healthcare systems and patients. A few studies evaluated the impact of various clinical and demographic variables on patients' length of stay (LOS). Hence, in this study, we aimed to investigate the impact of various variables on traumatic patients' LOS.

Road traffic injuries present a significant public health burden, especially in developing countries. This systematic review and meta-analysis synthesized global evidence on motorcycle helmet use prevalence by including 299 records across 249 articles involving 5,006,476 participants from 1982 to 2022. The findings revealed a declining trend in helmet use prevalence over the past four decades, with an overall prevalence of 48.

Article Synopsis
  • The study is a scoping systematic review aimed at summarizing biochemical markers found in cerebrospinal fluid (CSF) and serum related to spinal cord injury (SCI) in both humans and rats.
  • Researchers systematically searched multiple electronic databases and filtered through nearly 20,000 articles to finally review 45 relevant studies.
  • The findings indicate significant variability in biomarkers across different studies, but suggest that CSF and serum biomarkers are useful for assessing the severity of SCI based on their expression patterns and concentrations over time.

Article Synopsis
  • The study compared various scoring systems used for pediatric trauma to determine their effectiveness in predicting in-hospital mortality and ICU admissions among 6709 patients aged 18 and under.
  • Findings revealed that the adjusted trauma and injury severity score (aTRISS) was the best predictor of mortality, while the new trauma and injury severity score (NTRISS) excelled in predicting ICU admissions.
  • The results highlight the importance of using TRISS, aTRISS, and NTRISS in clinical settings to identify at-risk patients and improve outcomes.

Key Clinical Message: Tuberculosis (TB) is a rare but known reason for hypercalcemia usually in those with underlying conditions such as renal failure, diabetes, or severe anemia. It is essential to consider TB in those with refractory or resistant hypercalcemia.

Abstract: Hypercalcemia or a calcium level above 10.

Purpose: This study aimed to implement the Quality of Care (QoC) Assessment Tool from the National Spinal Cord/Column Injury Registry of Iran (NSCIR-IR) to map the current state of in-hospital QoC of individuals with Traumatic Spinal Column and Cord Injuries (TSCCI).

Methods: The QoC Assessment Tool, developed from a scoping review of the literature, was implemented in NSCIR-IR. We collected the required data from two primary sources.

: Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a rare multisystem autoimmune disease developed by autoantibody production against human neutrophilic granulocytes, including proteinase-3 (PR3) and myeloperoxidase (MPO). The management of AAV patients is difficult due to the multiorgan involvement, high rate of relapse, and complications of immunosuppressive agents that make it challenging. This study aims to investigate the efficacy and safety of rituximab (RTX) therapy in patients with granulomatosis with polyangiitis (GPA) or microscopic polyangiitis (MPA) subtypes.

This study was designed to determine the effective therapeutic parameters and evaluate the regenerative potential of low-level laser therapy (LLLT) after traumatic spinal cord injuries (TSCIs) in animal studies. The EMBASE and MEDLINE databases were searched on October 5, 2019, and followed with an update on January 2, 2021. All animal studies discussing the effect of LLLT on main pathophysiological events after TSCI, including inflammation, axon growth, remyelination, glial scar formation, cavity size, and locomotor recovery, were included.

Background: Granulomatosis with polyangiitis is a systemic anti-neutrophil cytoplasmic antibody-associated vasculitides (AAVs), mainly involving the respiratory tract and renal system. Treatment by Rituximab as a next-generation therapy in ANCA-associated vasculitis is associated with promising outcomes in GPA patients. Despite symptoms improvements, disease recurrence and drug reaction are a challenging topic nowadays.

Background: Since late 2019, SARS-CoV-2 which leads to coronavirus disease 2019 (COVID-19), has caused thousands of deaths. There are some pieces of evidence that SARS-CoV-2 genome could be re-detectable in recovered patients.

Methods: We performed a systematic review in the PubMed/Medline database to address the risk of SARS-CoV-2 recurrence.

Purpose: Macrophages play an important role in mediating damage after Spinal cord injury (SCI) by secreting macrophage migration inhibitory factor (MMIF) as a secondary injury mediator. We aimed to systematically review the role of MMIF as a therapeutic target after traumatic SCI.

Methods: Our systematic review has been performed according to the PRISMA 2009 Checklist.

Article Synopsis
  • Emerging evidence highlights the potential of biofluid-based biomarkers for diagnosing and predicting outcomes in traumatic brain injuries (TBI), but a structured understanding of available materials is lacking.
  • A systematic scoping review identified 1,036 original human studies out of over 25,000 records, revealing 540 biomarkers categorized into 19 distinct groups.
  • Key findings showed that cytokines, coagulation tests, and nerve tissue proteins were the most researched, but no single biomarker proved definitive, indicating a need for further exploration and combined biomarker panels for enhanced diagnosis and prognosis.
