Although bispecific T-cell engager (BiTE) is a promising treatment for relapsed/refractory multiple myeloma (RRMM), it needs to be evaluated in a real-world setting. This study aimed to evaluate the efficacy and safety of BiTEs compared with a synthetic standard of care (SOC). From a multicenter registry database of 474 patients with RRMM who received third- or more advanced-line treatments between January 2021 and October 2023, 1:1 propensity score-matched BiTE cohort (n = 71) and SOC cohort (n = 71) were established.

Background: Capsular contracture is one of the most common and severe complications after implant-based breast reconstruction. Recently, prepectoral implant-based breast reconstruction using acellular dermal matrix (ADM) has become an alternative to subpectoral implant-based reconstruction. However, risk factors for capsular contracture associated with recent prepectoral reconstruction trends are not well refined yet.

Article Synopsis
  • Finding the right drug dosage is essential and challenging in clinical trials, focusing on identifying the minimum effective dose (MED) in anesthesia and the maximum tolerated dose (MTD) in oncology.
  • The authors propose two new methods to improve dose estimation: reparametrized Firth's logistic regression (rFLR) and ridge-penalized reparametrized Firth's logistic regression (RrFLR), aimed at reducing bias in small sample sizes.
  • Numerical studies show that these methods outperform traditional approaches in mean squared error, bias reduction, and confidence interval accuracy, particularly in the up-and-down biased-coin design.

  • In breast surgeries, the nipple-areola complex can lead to contamination by normal bacteria, making it hard to keep the surgical area completely sterile.
  • This study focused on the bacterial profile of nipple flora in breast cancer patients who had implant-based reconstructions, linking the presence of bacteria to postoperative complications like capsular contracture.
  • Results showed that nearly 47% of patients had positive cultures for bacteria, with significant findings that certain bacteria, especially Staphylococcus epidermidis, were associated with postoperative infections, emphasizing the need for further research on preventive measures like antibiotic treatment.

Article Synopsis
  • - Myeloid-derived suppressor cells (MDSCs) are immunosuppressive cells originating from bone marrow, key in the tumor environment, but how they move from the bone marrow to tumors is not fully understood.
  • - The study found that activation of bone marrow stromal cells by parathyroid hormone (PTH) triggers the release of monocytic MDSCs from the bone marrow, but this does not increase their immunosuppressive function.
  • - Activation of PTH1R in osteoblasts leads to the production of VEGF-A and IL6, which affects MDSC behavior by triggering specific signaling pathways that result in MDSC detachment from osteoblasts, contributing to their mobilization.

  • An immediate lymphatic reconstruction (ILR) combining axillary reverse lymphatic mapping and lymphovenous anastomosis is being studied as a new surgical technique aimed at preventing upper extremity lymphedema in breast cancer patients after mastectomy.
  • A cohort study analyzed 213 patients, comparing 30 who underwent ILR with 183 who did not, to assess the effectiveness of this preventive treatment.
  • Results showed a significantly lower incidence of lymphedema in the ILR group (3.8%) compared to the control group (7.7%), indicating that ILR may help reduce the risk, but more extensive research is needed for validation.

Background: Mammography screening has been proven to detect breast cancer at an early stage and reduce mortality; however, it has low accuracy in young women or women with dense breasts. Blood-based diagnostic tools may overcome the limitations of mammography. This study assessed the diagnostic performance of a three-protein signature in patients with suspicious breast lesions.

Susceptible-exposed-infectious-recovered (SEIR) models were applied to assess the effectiveness of non-pharmaceutical interventions (NPIs) and to study the dynamic behavior of the COVID-19 pandemic. Recently, SEIR models have evolved to address the change of human mobility by some NPIs for predicting the new confirmed cases. However, the models have serious limitations when applied to Seoul.

  • The study aimed to evaluate how well prostate-specific antigen (PSA) and MRI can predict prostate cancer in patients with PSA levels of 20 ng/mL or less.
  • A total of 881 patients suspected of having prostate cancer underwent PSA tests and pre-biopsy MRIs, using their biopsy results or 2-year clinical follow-up data to confirm diagnoses.
  • The findings showed that while adding MRI to PSA improved specificity and positive predictive value for diagnosing cancer, it resulted in lower sensitivity, but still effectively reduced the need for unnecessary biopsies.

Machine learning (ML) has been suggested to improve the performance of prediction models. Nevertheless, research on predicting the risk in patients with acute myocardial infarction (AMI) has been limited and showed inconsistency in the performance of ML models versus traditional models (TMs). This study developed ML-based models (logistic regression with regularization, random forest, support vector machine, and extreme gradient boosting) and compared their performance in predicting the short- and long-term mortality of patients with AMI with those of TMs with comparable predictors.

  • Previous abdominal scars can complicate breast reconstruction using abdominal flaps by affecting tissue blood flow and weakening the abdominal fascia, leading to increased postoperative complications.* -
  • A systematic review of 11 studies involving over 2100 patients found that patients with previous scars had a higher overall risk of complications (RR 1.12) and donor-site complications (RR 1.35), especially significant in cases of wound issues (RR 1.83).* -
  • Proper preoperative planning, such as using CT angiography, can help manage these risks and improve outcomes despite the presence of prior scars.*
