Background: Surgical standardization for transverse colon cancers (TCC) has not been established, and the oncologic benefit of central vessel ligation (CVL) are still unclear. This study aimed to evaluate the oncologic safety of TCC surgery without CVL of the middle colic artery (MCA).

Methods: This is a single-center, retrospective, observational, comparative study.

Article Synopsis
  • This study aims to assess how the spread through air spaces (STAS) affects the prognosis of invasive mucinous adenocarcinoma (IMA) in patients post-surgery.
  • From 2015 to 2019, data from patients with complete IMA resection were analyzed, revealing that STAS was found in 39.1% of cases, correlating with larger tumors and greater lympho-vascular invasion.
  • The findings show that patients with STAS had significantly lower 5-year recurrence-free survival rates (66.1%) compared to those without STAS (91.8%), indicating that STAS is linked to a poorer prognosis and an increased risk of locoregional recurrence.

Background: We compared the clinical outcomes between endoscopic vacuum therapy (EVT) and conventional treatment (CT) for the management of post-esophagectomy anastomotic leakage.

Methods: A retrospective review of the medical records of patients who underwent esophagectomy with esophagogastrostomy from November 2003 to August 2021 was conducted. Thirty-four patients who developed anastomotic leakage were analyzed according to whether they underwent CT (n = 13) or EVT (n = 21).

Objectives: The purpose of this study was to explore the safety and feasibility of video-assisted thoracic surgery (VATS) total thymectomy via the single-port subxiphoid approach compared with the intercostal approach.

Methods: From January 2018 to May 2022, patients who underwent VATS total thymectomy via the subxiphoid or unilateral intercostal approach and diagnosed with Masaoka-Koga stage I-II, non-myasthenic thymoma were included in this study. Perioperative outcomes, immediate and long-term pain evaluations were compared in a propensity score-matching analysis.

Background: A combination of oral antibiotics and mechanical bowel preparation is recommended for patients scheduled to undergo elective colorectal surgery on the basis of recent large trials that have reported the superiority of this approach in preventing surgical site infections (SSIs). However, there are no standard recommendations for this approach. Therefore, in this study, we evaluated the efficacy of rifaximin and metronidazole and mechanical bowel preparation for preventing SSIs in cases of minimally invasive surgery for colorectal cancer.

Article Synopsis
  • The study aimed to explore how spinal alignment and genetic factors impact health-related quality of life in older adults with spinal deformities.
  • Researchers assessed sagittal parameters and muscle quality in 120 participants while using various functional scales to gauge disability and pain.
  • They found that specific misalignments in spinal curvature negatively affected quality of life, and identified a significant genetic variant linked to severe cases of adult spinal deformity, providing insights into its underlying causes.

Article Synopsis
  • The study aimed to assess how osteoporosis affects readmission rates, medical costs, and the risk of reoperation in patients over 50 who had spine surgery.
  • Data was collected from 147,676 patients in South Korea who underwent spine surgery from 2005 to 2008 and were followed for 8 years, comparing those with and without osteoporosis.
  • Results showed that patients with osteoporosis had higher readmission rates, longer hospital stays, and significantly increased medical costs, highlighting the importance of managing osteoporosis before spine surgeries to improve outcomes and reduce economic burden.

Article Synopsis
  • - Sagittal imbalance is a complex spinal deformity that can result from various factors like spinal stenosis and vertebral fractures, but research on its development is limited.
  • - The study followed 96 elderly participants with significant sagittal imbalance, eventually tracking 69 over two years to assess factors leading to further deterioration of their condition.
  • - Findings showed that 26% of participants experienced worsening sagittal imbalance, with a higher pelvic incidence and smaller multifidus muscle volume being critical factors contributing to this aggravation.

Background Context: Recombinant human bone morphogenic protein 2 (rhBMP2) has been used to induce bone fusion in patients with spinal fusion surgery. However, the effectiveness of rhBMP2 in the bone fusion process is limited in osteoporosis patients, and a high dose of rhBMP2 for enough bone fusion sometimes provokes side effects. Therefore, substitutes for rhBMP2 with a higher therapeutic potency are needed, and already several studies have published the effectiveness of Activin A/BMP2 chimera (AB204) in new bone formation process in vitro and in vivo.

Article Synopsis
  • The study aimed to compare the prevalence of proximal adjacent segment disease (ASD) versus distal ASD in patients who underwent thoracolumbar spinal fusion involving more than two levels.
  • Out of 856 cases reviewed, proximal ASD occurred in 4.32% of cases, while distal ASD occurred in 3.39%, showing a higher but not significantly different rate for proximal levels.
  • Key factors like the Pfirrmann grade and segmental range of motion (ROM) were identified as potential risk factors for the occurrence of ASD, particularly highlighting that increased proximal ROM correlates with a higher likelihood of developing proximal ASD.

We report an unusual case of delayed bleeding after open surgical repair of a thoracoabdominal aortic aneurysm. A 79-year-old man developed a massive retroperitoneal hematoma 49 days after Crawford type III thoracoabdominal aorta replacement. During emergency surgery, a tear was found in the prosthetic vascular graft caused by a sharp bony spur arising from the second lumbar vertebral body.

Background: This study investigated the clinical outcomes of surgical treatment of primary chest wall soft tissue sarcoma (CW-STS).

Methods: Thirty-one patients who underwent surgery for CW-STS between 2000 and 2015 were retrospectively reviewed. The disease-free and overall survival rates were estimated using the Kaplan-Meier method, and prognostic factors were analyzed using a Cox proportional hazards model.

Objective: To compare the spinal bone fusion properties of activin A/BMP2 chimera (AB204) with recombinant human bone morphogenetic protein (rhBMP2) using a rat posterolateral spinal fusion model.

Methods: The study was designed to compare the effects and property at different dosages of AB204 and rhBMP2 on spinal bone fusion. Sixty-one male Sprague-Dawley rats underwent posterolateral lumbar spinal fusion using one of nine treatments during the study, that is, sham; osteon only; 3.

To identify that the combined G-CSF and treadmill exercise is more effective in functional recovery after spinal cord injury (SCI). Rats were divided into 4 groups: a SCI group treated with G-CSF (G-CSF group,  = 6), a SCI group treated with treadmill exercise plus G-CSF (G-CSF/exercise group,  = 6), a SCI group with treadmill exercise (exercise group,  = 6), and a SCI group without treatments (control group,  = 6). We performed laminectomy at the T8-10 spinal levels with compression injury of the spinal cord in all rats.

Objective: The authors prospectively analyzed the effect of one-level or two-level anterior cervical discectomy and fusion (ACDF), comparing stand-alone cages and cage-with-plate fixation constructs with respect to clinical outcomes and radiologic changes.

Methods: A total of 84 patients who underwent one-level (n=52) or two-level ACDF (n=32) for cervical disc disease and who completed 2 years of follow-up were included in this study. The patients were divided by cervical level and grouped into ACDF-Cage-only and ACDF-Cage-with-plate groups.

Background: Cervical disc disease is a common and occasionally disabling condition, occurring as a natural consequence of aging in the vast majority of the adult population. Percutaneous epidural neuroplasty (PEN) has been used to deliver highly concentrated drugs for chronic neck pain and to prevent scarring in cases refractory to conventional epidural blocks. However, the clinical course after PEN in cervical disc disease is not well-documented.
