Article Synopsis
  • - Sagittal imbalance is a complex spinal deformity that can result from various factors like spinal stenosis and vertebral fractures, but research on its development is limited.
  • - The study followed 96 elderly participants with significant sagittal imbalance, eventually tracking 69 over two years to assess factors leading to further deterioration of their condition.
  • - Findings showed that 26% of participants experienced worsening sagittal imbalance, with a higher pelvic incidence and smaller multifidus muscle volume being critical factors contributing to this aggravation.

Background Context: Recombinant human bone morphogenic protein 2 (rhBMP2) has been used to induce bone fusion in patients with spinal fusion surgery. However, the effectiveness of rhBMP2 in the bone fusion process is limited in osteoporosis patients, and a high dose of rhBMP2 for enough bone fusion sometimes provokes side effects. Therefore, substitutes for rhBMP2 with a higher therapeutic potency are needed, and already several studies have published the effectiveness of Activin A/BMP2 chimera (AB204) in new bone formation process in vitro and in vivo.

Article Synopsis
  • The study aimed to compare the prevalence of proximal adjacent segment disease (ASD) versus distal ASD in patients who underwent thoracolumbar spinal fusion involving more than two levels.
  • Out of 856 cases reviewed, proximal ASD occurred in 4.32% of cases, while distal ASD occurred in 3.39%, showing a higher but not significantly different rate for proximal levels.
  • Key factors like the Pfirrmann grade and segmental range of motion (ROM) were identified as potential risk factors for the occurrence of ASD, particularly highlighting that increased proximal ROM correlates with a higher likelihood of developing proximal ASD.
