Article Synopsis
  • The study aimed to assess how familial Mediterranean fever (FMF) affects the phenotype and progression of Crohn's disease (CD) in an area where FMF is common.
  • Out of 210 CD patients, a small percentage (3.8%) also had FMF, which is much higher than what is typically seen, indicating a strong correlation in this region.
  • Patients with both conditions experienced higher rates of peripheral arthritis and a greater need for biological therapy, suggesting a more severe CD course when FMF is present.

Background And Study Aims: Self expandable metal stents (SEMS) is an alternative to emergency surgery to treat malignant large bowel obstruction. It can be used either for palliation or as a bridge to curative surgery. Our study aims to review the outcomes of SEMS treatment in a tertiary center and to find predictors for the clinical outcome.

Solitary rectal ulcer syndrome (SRUS) is a rare disorder likely to be caused by a traumatic and ischemic mucosal injury related to latent or overt rectal prolapse. Mucosal damage can vary between erythema and up to intractable ulceration. The typical symptoms of SRUS may resemble those of inflammatory bowel disease (IBD)-related proctitis or rectal malignancy with mucus- and blood-mixed defecation with urgency and sometimes transient incontinence.

Article Synopsis
  • Crohn's disease (CD) involves ongoing inflammation of the gastrointestinal tract, often needing imaging tests for assessment.
  • The study aimed to compare the effectiveness of intestinal ultrasound (IUS) and magnetic resonance enterography (MRE) in measuring CD activity, using a simple endoscopic score as a benchmark.
  • Both IUS and MRE performed similarly for detecting general activity, but MRE was better at identifying certain lesions, while IUS showed higher specificity and sensitivity for ascites, suggesting it may be a better option for monitoring CD.

Background: Liver biopsy is an invasive procedure that is currently still necessary for predicting underlying hepatic injury related to chronic viral hepatitis B (CVHB). To date, none of the studied non-invasive methods have been able to replace liver biopsy. An apoptotic serum marker, M30, which has been reported to indicate ongoing liver fibrosis, has been popular in recent years.

Article Synopsis
  • Pantoprazole, a proton pump inhibitor, is crucial for treating peptic ulcer disease (PUD) and is metabolized by CYP enzymes, specifically CYP2C19 and CYP3A4.
  • Genetic variations (SNPs) in these enzymes and multi-drug resistance protein 1 (MDR1) can influence how effectively pantoprazole works by changing its plasma levels.
  • In a study of 194 patients with H. pylori-related conditions, CYP2C19 polymorphisms impacted pantoprazole concentrations, but CYP3A4 and MDR1 variations did not significantly affect the success rate of H. pylori eradication.

Article Synopsis
  • The study aimed to explore the connection between early atherosclerosis and inflammatory bowel disease (IBD) through various markers.
  • It involved 96 IBD patients (58 with ulcerative colitis, 36 with Crohn's disease) and 65 healthy controls, comparing several metrics like carotid intima-media thickness (CIMT) and levels of von Willebrand factor antigen (VWF-Ag) and D-dimer.
  • Results indicated that IBD patients had higher VWF-Ag and D-dimer levels than controls, suggesting these markers may help identify early atherosclerosis in IBD patients, even though other markers showed no significant differences.

Article Synopsis
  • HCC (Hepatocellular Carcinoma) is the most frequent primary liver tumor and requires high-risk HBV carriers to undergo surveillance through ultrasound every 6-12 months as per AASLD guidelines.
  • The serum α-fetoprotein level is used for diagnosing HCC but can also be elevated in other conditions like acute hepatitis, cirrhosis, and various cancers, which complicates the diagnosis.
  • Clinicians must be cautious and consider other potential causes for elevated α-fetoprotein levels when assessing patients with liver masses, particularly in those with HBV-related cirrhosis.

Background: Dieulafoy's lesion (DL) is a relatively uncommon medical condition characterized by a large tortuous arteriole in the submucosa of any part of gastrointestinal (GI) tract wall that bleeds via erosion likely caused in the submucosal surface by protrusion of the pulsatile arteriole. Compared with other endoscopic hemostatic techniques, clipping alone for DL is limited.

Aims: The aim of the present case series study is to identify common clinical and endoscopic features, rates of occurrence, to review the outcome of endoscopic management of upper GI tract DL, and to illustrate the use and the efficiency of endoclips in maintaining the GI bleeding due to DL.

Colorectal cancer (CRC) is the third most common cause of cancer-related death in Europe. The aim of the present study was to elucidate the efficiency of the neutrophil/lymphocyte ratio (NLR), the platelet/lymphocyte ratio (PLR), and the mean platelet volume (MPV) as tools for the preoperative diagnosis of CRC and their usefulness in the follow-up of CRC. A total of 144 CRC patients, as diagnosed by colonoscopy, and 143 age-matched and sex-matched healthy participants were included in the study.

Background: The values of C-reactive protein (CRP) can prove useful in determining disease progress. Because of synthesis by the liver, production of CRP in response to inflammation may be attenuated in patients with liver dysfunction. This may result in differences interpreting CRP levels in patient with portal and non-portal hypertension ascites.
