There are many aspects in the relationship between smoking and sleep that have not been investigated thoroughly yet, especially in regards to obstructive sleep apnea-hypopnea syndrome (OSAHS). In this cross-sectional study, 2359 participants, who have visited the sleep clinic of our hospital during a 13-year period and were former or current smokers, were included. Their smoking history, measured in packyears of smoking, and their nicotine dependence, measured with the Fagerström scale, were correlated with various epidemiological and sleep-related variables.

  • The report discusses a case of a 48-year-old patient with pre-extensively drug-resistant tuberculosis (XDR tb) who died from myocarditis following a complex treatment regimen.
  • The patient's history of poor adherence to medication resulted in acquired resistance to both first- and second-line drugs, complicating her condition.
  • This case highlights a potentially lethal side effect of drug treatment for resistant tb, urging clinicians to be vigilant about the risk of drug-induced myocarditis in similar patients.

In this review, two cases of testicular tuberculosis (TB) are presented, and another 58 cases published in PubMed between January 1, 2012, and July 31, 2023, are reviewed. Testicular TB remains a disease mainly of the developing world, with one notable exception - the infections caused as a result of Bacillus Calmette-Guérin infusion immunotherapy for bladder cancer. Its clinical course is subacute; however, it might get disseminated and become life-threatening; therefore, prompt diagnosis is very important.

COVID-19 might present with a wide range of clinical manifestations, from mild respiratory distress to severe multi-organ dysfunction. We present a unique case of complex COVID-19 presentation in a 45-year-old female who initially developed general symptoms such as fever, cough, headache, and weakness, which escalated to coma, requiring intubation and ICU admission. A brain MRI revealed lesions compatible with encephalitis, the cause of which remained unexplained after an in-depth clinical, laboratory, and imaging investigation.

  • Tracheostomy is a procedure often used for patients in ICUs who need long-term ventilation or have airway obstructions, but conflicting studies exist on its timing and benefits.
  • This study analyzed data from 660 critically ill patients over two years, examining various factors like demographics, illness severity, tracheostomy timing, and outcomes.
  • Results indicated that patients who had early tracheostomies experienced significantly shorter durations on mechanical ventilation and in the ICU compared to those with late procedures, along with preferences for specific techniques based on patient conditions.

  • Tobacco smoking increases the risk of cardiovascular diseases (CVD) and is linked to obstructive sleep apnea (OSA), both of which contribute to significant health issues.
  • There is limited research on how pharmacotherapy for smoking cessation, like nicotine replacement therapy (NRT), bupropion, and varenicline, impacts OSA and cardiovascular health, with some studies showing mixed results.
  • Personalized treatment is crucial for smokers with OSA during cessation attempts, and more research is needed to assess the effectiveness and safety of smoking cessation medications in this population.

  • Hepatic ischemia-reperfusion syndrome causes complications after surgeries like liver transplants and major resections, impacting not just the liver but also other body systems, prompting the need for better treatments.
  • Treprostinil, a synthetic form of prostaglandin I2, has shown promising results in reducing injury during liver transplantation and is already approved for treating pulmonary arterial hypertension.
  • It helps improve liver and kidney function while reducing inflammation and oxidative stress, suggesting it could be an effective therapy to minimize damage from ischemia-reperfusion injury.

Long-acting muscarinic antagonists (LAMAs) are a class of inhalers that has recently been included as add-on therapy in the GINA guidelines, either in a single inhaler device with inhaled corticosteroids plus long-acting β2-agonists (ICS + LABA) (closed triple inhaler therapy) or in a separate one (open triple inhaler therapy). This review summarizes the existing evidence on the addition of LAMAs in patients with persistently uncontrolled asthma despite ICS + LABA treatment based on clinical efficacy in the reduction of asthma symptoms and exacerbations, the improvement in lung function, and its safety profile.

  • - Invasive pulmonary aspergillosis (IPA) is a significant concern for critically ill COVID-19 patients, but accurately measuring its prevalence and effects is complicated due to inconsistent symptoms and varying clinical practices.
  • - Diagnosing COVID-19-associated pulmonary aspergillosis (CAPA) relies on specific tests (like serum and BAL GM tests), as traditional methods often yield unreliable results, and bronchoscopy should be reserved for cases that would change treatment.
  • - Effective management of CAPA aims to enhance survival by preventing misdiagnosis and ensuring prompt antifungal treatment, considering factors like infection severity and potential drug interactions, though the best length of treatment is still debated.

  • The study examined the impact of Elexacator/tezacaftor/ivacaftor (ETI) on the management of cystic fibrosis (CF) in 71 adult patients over one year.
  • Results showed a significant decrease in the use of inhaled medications like dornase-α and several antibiotics after starting ETI, indicating improved health outcomes.
  • The authors suggest that while these findings are promising, further research through larger clinical trials is necessary to fully understand the long-term effects of ETI on CF treatment.

  • Sexualised behaviour in sleep (SBS) is a rare condition that involves sexual actions occurring during non-REM sleep, with limited research on its characteristics and connections to other psychological or sleep disorders.
  • A study analyzed data from 335 patients over 15 years, identifying 65 with SBS, primarily adult males, and found SBS often starts in adulthood while other NREM disorders typically begin in childhood.
  • Key factors triggering SBS include alcohol use, relationship issues, and sleep deprivation, with SBS patients more likely to report sleepwalking alongside their symptoms.

  • - New coronavirus SARS-CoV-2 emerged in December 2019 in Wuhan, China, leading to a global COVID-19 pandemic, primarily presenting with respiratory symptoms but also causing significant liver injuries.
  • - Liver damage in COVID-19 patients can range from mild enzyme elevation to severe injuries and is caused by various factors like the virus's direct effects, an overactive immune response, and complications from underlying liver conditions.
  • - The review focuses on the prevalence and mechanisms of liver injury in COVID-19 patients, especially those with pre-existing liver diseases, and discusses important considerations for their management during the pandemic.

Omalizumab is the first monoclonal antibody that was globally approved as a personalized treatment option for patients with moderate-to-severe allergic asthma. This review summarizes the knowledge of almost two decades of use of omalizumab to answer some important everyday clinical practice questions, concerning its efficacy and safety and its association with other asthma-related and drug-related parameters. Evidence suggests that omalizumab improves asthma control and reduces the incidence and frequency of exacerbations in patients with severe allergic asthma.

  • The study evaluated how well teenagers and adults with cystic fibrosis (CF) adhered to inhaled therapies over four years and examined the impact of adherence on health outcomes.
  • Results indicated that the average medication possession ratio was 0.75, with 43.4% of participants showing consistent adherence; those with stable adherence had significantly better medication management and weight outcomes.
  • The findings suggest that better adherence is linked to higher patient weight, and the authors call for further research to understand the relationship between adherence and health in CF patients.

Mechanically ventilated patients typically exhibit abnormal respiratory sounds. Squawks are short inspiratory adventitious sounds that may occur in patients with pneumonia, such as COVID-19 patients. In this work we devised a method for squawk detection in mechanically ventilated patients by developing algorithms for respiratory cycle estimation, squawk candidate identification, feature extraction, and clustering.
