Myotomy or myectomy are well known as the standard treatment of hypertrophic obstructive cardiomyopathy (HOCM), and mitral valve replacement (MVR) is reported to achieve the equivalent therapeutic effect. And recently, combined treatment with artificial chordae replacement and MVR has been reported to improve the prognosis. We herein describe a case of a patient with HOCM who developed acute exacerbation of heart failure.

Article Synopsis
  • A 57-year-old man showed a concerning mass on a chest x-ray, which grew larger over the course of a year.
  • A lung cancer suspicion was raised following a PET scan, leading to surgery (lingular segmentectomy).
  • The final diagnosis revealed the mass to be pulmonary actinomycosis instead of cancer, highlighting the importance of considering this condition as a possible lung cancer alternative.

Spontaneous nonaneurysmal rupture of the aorta is a life-threatening condition for which emergency diagnostic and therapeutic measures are indicated. An asymptomatic spontaneous aortic rupture was unexpectedly discovered adjacent to the visceral aorta. We diagnosed the mass as a pseudoaneurysm, and surgery was performed.

Surgical treatments of aortic root involvement in aortitis syndrome are frequently complicated by valve detachment and pseudoaneurysmal formation during active inflammation. A 40-year-old woman with aortitis syndrome complicating the ascending aortic aneurysm, severe aortic regurgitation (AR), and left coronary ostial stenosis was successfully treated by aortic root replacement and concomitant coronary artery bypass grafting (CABG) during acute inflammation. We devised an intravalvular implantation between the fragile aortic annulus and Teflon felt to a modified Bentall's procedure, which prevented anastomotic leakage and pseudoaneurysmal formation in the late period.
