: Although lateral mass screws lower the risk of vertebral artery injury, they are shorter and have a weaker purchase than a pedicle screw, thereby posing the risk of mechanical failure following a ≥3-level posterior cervical fusion (PCF). Therefore, the purpose of this study is to demonstrate that the posterior en bloc fusion of C2-7 is mechanically stronger than shorter, regionally confined posterior fusions of the cervical spine. : We included 178 patients who underwent PCF with ≥3 levels.

Radiotherapy is one of the various treatment options for patients with metastatic spinal tumors (MST). However, it is difficult to say that this is definitely an optimal treatment for MST, and there are several patients who need surgical treatment because pain or neurologic deficits occur even after radiotherapy. Therefore, this study aimed to analyze which patients received early operative treatment after radiotherapy.

Background: A 24-item early-onset scoliosis questionnaire (EOSQ-24) has been developed as a valid tool for assessing the physical and emotional function of patients with early-onset scoliosis (EOS). Previous studies that conducted transcultural adaptation of the original EOSQ-24 into other languages have demonstrated the high reliability of the questionnaire. However, a Korean version of the EOSQ-24 is not available, limiting optimal patient assessment in this nation.

Background Context: Much controversy exists about whether posterior longitudinal ligament (PLL) resection should be performed during cervical artificial disc replacement (ADR). The PLL can be resected or preserved during the ADR procedure based on the shape and location of the compressive pathology. However, unlike fusion operations, the outcomes of ADR may be affected by PLL resection since segmental motion is preserved with ADR and the PLL restricts flexion of the segment.

Article Synopsis
  • Cervical myelopathy, a condition affecting the spinal cord in the neck, is commonly treated with either atlantoaxial fusion (AAF) or occipitocervical fusion (OCF), though there hasn't been a comprehensive study comparing their outcomes.
  • The goal of this research was to evaluate and compare the clinical and radiological results of AAF versus OCF over a minimum follow-up period of 2 years, involving 64 patients.
  • Results indicated that while both surgical techniques had similar overall outcomes in terms of neck pain and disability scores, patients who underwent OCF experienced significantly greater declines in lifting function and neck range of motion compared to the AAF group after the surgery.

Article Synopsis
  • The study aimed to identify risk factors for local recurrence after surgery in patients with metastatic spinal cord compression (MSCC), as this had not been well researched before.
  • A retrospective analysis of 304 patients revealed that local recurrence occurred in 16.4% of cases, with higher rates linked to certain surgical methods, tumor types, and the presence of multiple metastases or pathologic fractures.
  • Key findings indicated that decompression alone had a significantly higher recurrence rate compared to other surgical techniques, and factors like renal cell carcinoma, extensive spinal metastasis, and smoking were also associated with higher local recurrence risk.

Objective: Cord compression by the ligamentum flavum (CCLF) has been reported to adversely affect the clinical outcomes of anterior cervical discectomy and fusion (ACDF). While indirect decompression does occur for foraminal stenosis with ACDF, whether ACDF could improve CCLF with the distraction of disc space remains unclear. This study aimed to identify 1) whether indirect decompression occurs for CCLF with ACDF, and 2) risk factors that hinder the improvement of CCLF.

Article Synopsis
  • This study is a retrospective cohort analysis examining the use of a technique called guttering during anterior cervical discectomy and fusion (ACDF) in patients with cervical myelopathy.
  • The main goal was to see if guttering could effectively decompress retro-corporeal pathologies that traditional methods couldn't address, comparing outcomes between patients who underwent guttering and those who did not.
  • Findings showed no significant differences in fusion rates, pain levels, or neck disability after one year between the two groups, indicating that guttering could safely enhance the surgical approach without increasing complications.

Study Design: Retrospective cohort study.

Objective: To clarify whether clinical outcomes of anterior cervical discectomy and fusion (ACDF), is affected by presence of spinal canal-cord mismatch (SCCM).

Summary Of Background Data: SCCM is considered a factor that would moderately influence surgeons to perform posterior surgery since it could widen the spinal canal, while an anterior approach could only remove degenerative pathologies grown into the spinal canal.

Objective: Nonunion and significant subsidence after anterior cervical discectomy and fusion (ACDF) are associated with poor clinical outcomes, which occasionally lead to revision surgery. Allograft and polyetheretherketone (PEEK) cages are the two most commonly used interbody spacer devices for ACDF. Although studies have been conducted to compare the efficacies of these two interbody materials, the question remains regarding the superiority of one over the other.

Purpose: This study was conducted to develop a convolutional neural network (CNN) algorithm that can diagnose cervical foraminal stenosis using oblique radiographs and evaluate its accuracy.

Materials And Methods: A total of 997 patients who underwent cervical MRI and cervical oblique radiographs within a 3-month interval were included. Oblique radiographs were labeled as "foraminal stenosis" or "no foraminal stenosis" according to whether foraminal stenosis was present in the C2-T1 levels based on MRI evaluation as ground truth.

Objective: Radiotherapy is one of the important treatment options for metastatic spinal tumors but is not the definite intervention in all cases, as there are patients who still require surgical treatment because of severe pain or neurologic events after this treatment. We evaluated the perioperative effects of preoperative radiotherapy in these cases as a future guide for surgeons on critical considerations in this period.

Methods: We included 328 patients in this study who had undergone decompression and fusion surgery for metastatic spinal tumors.

Background Context: Adjacent segment degeneration (ASD) following lumbar fusion operation is common and can occur at varying timepoints after index surgery. An early revision operation for ASD, however, signifies a short symptom-free period and might increase the risk of successive surgeries.

Purpose: We aimed to elucidate the overall risk factors associated with revision surgeries for ASD with distinct attention to early revisions.

Background: Radiation therapy (RT) performed before anterior cervical spine surgery (ACSS) may cause fascial plane fibrosis, decreased soft-tissue vascularity, and vertebral body weakness, which could increase the risk of esophageal and major vessel injuries, wound complications, and construct subsidence. Therefore, this study aimed to evaluate whether preoperative RT performed for metastatic spine cancer (MSC) at the cervical spine increases perioperative morbidity for ACSS.

Methods: Forty-nine patients who underwent ACSS for treatment of MSC at the cervical spine were retrospectively reviewed.

The combination of X-ray free-electron lasers (XFELs) with serial femtosecond crystallography represents cutting-edge technology in structural biology, allowing the study of enzyme reactions and dynamics in real time through the generation of `molecular movies'. This technology combines short and precise high-energy X-ray exposure to a stream of protein microcrystals. Here, the XFEL structure of carbonic anhydrase II, a ubiquitous enzyme responsible for the interconversion of CO and bicarbonate, is reported, and is compared with previously reported NMR and synchrotron X-ray and neutron single-crystal structures.

Purpose: The S2AI screw technique has several advantages over the conventional iliac screw fixation technique. However, connecting the S2AI screw head to the main rod is difficult due to its medial entry point. We introduce a new technique for connecting the S2AI screw head to a satellite rod and compare it with the conventional method of connecting the S2AI screw to the main rod.

  • Attempts to develop drugs targeting the corticotropin-releasing hormone receptor 1 (CRFR) for stress-related therapy have not been successful so far.
  • High-resolution studies using X-ray free-electron lasers (XFELs) aided in understanding GPCR structures, leading to the elucidation of the CRFR's crystal structure with a specific antagonist.
  • Two new antagonists, BMK-C203 and BMK-C205, were developed, with BMK-C205 showing significant antidepressant effects in mice, suggesting potential for effective drug development against CRFR.

The photoisomerization reaction of a fluorescent protein chromophore occurs on the ultrafast timescale. The structural dynamics that result from femtosecond optical excitation have contributions from vibrational and electronic processes and from reaction dynamics that involve the crossing through a conical intersection. The creation and progression of the ultrafast structural dynamics strongly depends on optical and molecular parameters.
