Article Synopsis
  • The study investigates two home care models for elderly patients: an Integrated Model (IM) as a control and a Functional Model (FM) as a study group, focusing on their impact on resource use and hospital admissions.
  • It involves a two-year follow-up of over 120 patients aged 65 and older in Badalona, Spain, measuring health status, quality of care, and resource utilization through various assessments.
  • The results show that the Functional Model (FM) significantly reduces hospital admissions and the length of stays compared to the Integrated Model (IM), indicating improved continuity of care and potential cost savings.

Identifying mortality risk factors in people living in nursing homes could help healthcare professionals to individualize or develop specific plans for predicting future care demands and plan end-of-life care in this population. This study aims to identify mortality risk factors in elderly nursing home (NH) residents, based on variables adapted to this environment, routinely collected and easily accessible to their healthcare professionals. A prospective, longitudinal, observational study of NH residents aged 65 years and older was carried out collecting sociodemographic, functional and cognitive status, nutritional variables, comorbidities, and other health variables.

Background: Preventive home visits are suited for patients with reduced mobility, such as older people. Healthcare needs for older patients are expected to increase due to the extended life expectancy estimated in coming years. The implementation of low-cost, patient-centered methodologies may buffer this rise in health care costs without affecting the quality of service.
