Rationale: While thrombolysis is standard of care for patients with acute ischemic stroke (AIS) within 4.5 h of symptom onset, the benefit of tenecteplase beyond this time window is less certain.

Aim: The TIMELESS trial (NCT03785678) aims to determine if treatment with tenecteplase increases the proportion of good clinical outcomes among patients with stroke due to a large vessel occlusion who present beyond 4.

Extracorporeal Albumin Dialysis in Liver Failure with MARS and SPAD: A Randomized Crossover Trial.

Introduction: Liver failure is associated with hepatic and extrahepatic organ failure leading to a high short-term mortality rate. Extracorporeal albumin dialysis (ECAD) aims to reduce albumin-bound toxins accumulated during liver failure. ECAD detoxifies blood using albumin dialysis through an artificial semipermeable membrane with recirculation (molecular adsorbent recirculating system, MARS) or without (single-pass albumin dialysis, SPAD).

Effectiveness of pulmonary rehabilitation in COVID-19 respiratory failure patients post-ICU.

Introduction: Some COVID-19 patients develop respiratory failure requiring admission to intensive care unit (ICU). We aim to evaluate the effects of pulmonary rehabilitation (PR) post-ICU in COVID-19 patients.

Methods: Twenty-one COVID-19 patients were evaluated pre- and post-PR and compared retrospectively to a non-COVID-19 group of 21 patients rehabilitated after ICU admission due to respiratory failure.

Desulfovibrio species are representatives of microorganisms at the boundary between anaerobic and aerobic lifestyles, since they contain the enzymatic systems required for both sulfate and oxygen reduction. However, the latter has been shown to be solely a protective mechanism. By implementing the oxygen-driven experimental evolution of Desulfovibrio vulgaris Hildenborough, we have obtained strains that have evolved to grow with energy derived from oxidative phosphorylation linked to oxygen reduction.

The relationship between basic audio quality and overall listening experience.

Basic audio quality (BAQ) is a well-known perceptual attribute, which is rated in various listening test methods to measure the performance of audio systems. Unfortunately, when it comes to purchasing audio systems, BAQ might not have a significant influence on the customers' buying decisions since other factors, like brand loyalty, might be more important. In contrast to BAQ, overall listening experience (OLE) is an affective attribute which incorporates all aspects that are important to an individual assessor, including his or her preference for music genre and audio quality.

Applicability of Pulse Pressure Variation during Unstable Hemodynamic Events in the Intensive Care Unit: A Five-Day Prospective Multicenter Study.

Pulse pressure variation can predict fluid responsiveness in strict applicability conditions. The purpose of this study was to describe the clinical applicability of pulse pressure variation during episodes of patient hemodynamic instability in the intensive care unit. We conducted a five-day, seven-center prospective study that included patients presenting with an unstable hemodynamic event.

Transtracheal esophageal stent removal: a case-series.

Benign esophagorespiratory fistula is a rare but often lethal affection and difficult to cure. Possible treatments are surgery or esophageal stenting but may fail and cause respiratory failure. Two patients with spontaneous esophagorespiratory fistula after chemoradiotherapy for an esophageal malignancy were both treated by esophageal exclusion but esophageal stent were left in place.

[Profile and motivations of students performing a universitary degree in 2011].

Objective: Observational study relating to the inscriptions with universitary diplomas (DU) in relation with anesthesiology and intensive care during the year 2010-2011. We had in mind to know the profile and the motivations of the students concerned.

Method: A questionnaire was put on line and the registered students with the various diplomas were contacted by e-mail.

Prescriptions analysis by clinical pharmacists in the post-operative period: a 4-year prospective study.

Background: Clinical pharmacists can help prevent medication errors. However, data are scarce on their role in preventing medication prescription errors in the post-operative period, a high-risk period, as at least two prescribers can intervene, the surgeon and the anesthetist. We aimed to describe and quantify clinical pharmacist' intervention (PIs) during validation of drug prescriptions on a computerized physician order entry system in a post-surgical and post-transplantation ward.

Effects of a continuous low-dose clonidine epidural regimen on pain, satisfaction and adverse events during labour: a randomized, double-blind, placebo-controlled trial.

Background And Objective: Epidural clonidine has been proposed as an adjunct for anaesthetic mixtures during labour. Administered as a bolus, clonidine may have side effects such as sedation and hypotension; its continuous infusion could be attractive in this respect. We, therefore, conducted a randomized, double-blind trial using patient-controlled epidural analgesia with a background infusion using a low dose of clonidine during labour.

Factors associated with noninvasive ventilation failure in postoperative acute respiratory insufficiency: an observational study.

Background And Objective: Few data are available on the efficacy of noninvasive ventilation (NIV) in postoperative patients with acute respiratory failure (ARF).

Methods: Seventy-two patients coming from the surgical wards with postoperative ARF were retrospectively evaluated. The major characteristics of patients who were intubated were compared with the characteristics of those who were not after a trial of NIV.

[Management of patients with hepatitis C by French psychologists and psychiatrists: a practical investigation].

Introduction: A first part of this survey showed that up to 19% of patients with hepatitis C virus did not receive optimal treatment because of a psychiatric problem (psychiatric disorders or addictive behaviours) and that less than 50% of the managing clinicians worked with a psychiatrist or a psychologist in these cases. The aim of the second part of the survey was to describe the current practices provided by the psychiatrists and the psychologists to HCV infected patients as well as the knowledge and expectations of professionals in this area.

Patients And Methods: Forty-seven psychiatrists and 43 psychologists who were or were not working with French reference poles for hepatitis C virus responded to a questionnaire of their practices.

Temperature and deactivation of microbial faecal indicators during small scale co-composting of faecal matter.

Small scale co-composting of faecal matter from dry toilet systems with shredded plant material and food waste was investigated in respect to heat development and deactivation of faecal indicators under tropical semiarid conditions. Open (uncovered) co-composting of faecal matter with shredded plant material alone did not generate temperatures high enough (<55 degrees C) to reduce the indicators sufficiently. The addition of food waste and confinement in chambers, built of concrete bricks and wooden boards, improved the composting process significantly.

Management of psychiatric disorders and addictive behaviors in patients with viral hepatitis C in France.

Introduction: Guidelines concerning the management of psychiatric disorders and addictive behaviors in patients with chronic hepatitis C and the conditions of collaboration between hepatogastroenterologists, infectiologists, psychiatrists and psychologists have not been published. This has a negative influence on optimal therapeutic management of chronic hepatitis C virus (HCV) infection. The aim of this study was to describe the current clinical practices for ambulatory management of psychiatric disorders and addictions, and the influence of a possible psychiatric and/or psychological collaboration.

Acute fatal pulmonary embolism during cyanoacrylate injection in gastric varices.

We report a case of massive pulmonary embolism during cyanoacrylate glue endoscopic injection in a patient with gastric varices from portal hypertension. A review of the literature and results in an animal model show the physiopathology and risk factors associated with this endoscopic procedure.

[Increased troponin I level in a Duchenne muscular dystrophy patient with normal coronarography].

Duchenne muscular dystrophy (DMD) is a neuromuscular disease characterized by a progressive skeletal muscular weakness. As the respiratory care of such patients has been dramatically improved during the past few years, the DMD associated cardiomyopathy is becoming a new concern. We report a clinical case of a Troponin I level increase with normal coronarography occurring after an acute episode of respiratory failure.

Transfemoral venous access through inferior vena cava filters for interventions requiring large sheaths.

The presence of a filter in the inferior vena cava (IVC) to prevent pulmonary embolism from lower extremity deep vein thrombosis has traditionally been a contraindication for venous catheterization from the femoral vein. Certain interventional procedures require femoral access and occasionally, patients with IVC filters require these procedures. The authors report two such patients, one with a Greenfield filter requiring balloon mitral valvuloplasty and one with a Trapease filter requiring atrial septostomy.

[Case report: a bacteremic pasteurellosis].

Bacteremic pasteurellosis is an uncommon form of Pasteurella multocida infection, usually involved in local infections. This systemic infection often occurs in immuno-compromised patient such as cirrhotic or alcoholic patients, with a high mortality rate (up to 60%). Septic shock may occur and neurological disorders or coma are frequent.

[Anaesthesia for a non cardiac surgery in a patient with an Eisenmenger syndrome. Interest of a non invasive haemodynamic monitoring].

Patients with an Eisenmenger syndrome have an instuble hemodynamic status. During a general anaesthesia, the intracardiac shunt has to maintain the correct orientation and volume, adapted to each patient, in such a condition, to avoid the risk of hypoxemia and cardiac failure. The haemodynamic monitoring with a Swan Ganz catheter could be useful.
