Laryngeal complications after thyroidectomy: is it always the surgeon?

Hypothesis: Laryngeal dysfunction after thyroidectomy is a common complication. However, few data are available to differentiate whether these complications result from injury to the recurrent nerve or to the vocal folds from intubation.

Setting: University medical center.

Reduced neorectal capacitance is a more important factor for impaired defecatory function after rectal resection than the anal sphincter pressure.

Objective: The role of the diverse anorectal diagnostic tools like manometry and determination of the preception threshold and the maximal tolerable volume is still a matter of debate. Currently, there is a scarcity of physiological data in the long-term follow-up of patients who underwent sphincter-preserving rectal resection. The aim of this study was therefore to perform these anorectal physiological measurements and to correlate the determined parameters with a faecal incontinence score.

Inhibition of hemoxygenase-1 improves survival after liver resection in jaundiced rats.

Background: The role of hemoxygenase (HO)-1 after partial liver resection (PLR) in jaundiced animals has yet to be defined. We therefore investigated: (1) the acute effects of bile duct ligation (BDL) on bilirubin accumulation and hepatocellular integrity after PLR; (2) how BDL and PLR affect HO-1 protein expression; (3) how functional HO-1 blockade affects survival and liver regeneration after BDL and PLR.

Methods: Male Sprague-Dawley rats were subjected to BDL or a sham operation.

Extraction of SSVEP signals of a capacitive EEG helmet for human machine interface.

The use of capacitive electrodes for measuring EEG eliminates the preparation procedure known from classical noninvasive EEG measurements. The insulated interface to the brain signals in combination with steady-state visual evoked potentials (SSVEP) enables a zero prep human machine interface triggered by brain signals. This paper presents a 28-channel EEG helmet system based on our capacitive electrodes measuring and analyzing SSVEPs even through scalp hair.

Rho-kinase inhibitor attenuates cholestasis-induced CXC chemokine formation, leukocyte recruitment, and hepatocellular damage in the liver.

Background: In the present experimental study, we analyzed the role of Rho-kinase during obstructive cholestasis by studying the effect of the Rho-kinase inhibitor Y-27632 on hepatic CXC chemokine formation, leukocyte recruitment and hepatocellular damage.

Materials And Methods: C57BL/6 mice underwent bile duct ligation (BDL) to induce obstructive cholestasis. Mice were pretreated with Y-27632 (1 and 10mg/kg) or the vehicle PBS.

[Advance health care directives as seen by surgical patients].

Background: Progressive health care implies progress also in physician/patient interaction, especially with regard to moribund patients and their relatives. Advance health care directives emerged from the desire to influence medical treatment even in borderline situations. In spite of the present political and public discussions in Germany, advance directives are rarely of much importance in everyday surgical practice.

The effects of feeding distillers dried grains with solubles on broiler meat quality.

A randomized complete block design with 3 replications (n=144) was utilized to evaluate the effects of feeding distillers dried grains with solubles (DDGS; 0% control and 8%) on broiler breast and thigh meat quality. Electrical stunning was performed, and broiler carcasses were scalded, picked, and eviscerated using commercial prototype equipment. At 4 h postmortem, carcasses were removed from the chill tank and breast and thigh removal was performed.

Translating research into practice intervention improves management of acute pain in older hip fracture patients.

Objective: To test an interdisciplinary, multifaceted, translating research into practice (TRIP) intervention to (a) promote adoption, by physicians and nurses, of evidence-based (EB) acute pain management practices in hospitalized older adults, (b) decrease barriers to use of EB acute pain management practices, and (c) decrease pain intensity of older hospitalized adults.

Study Design: Experimental design with the hospital as the unit of randomization.

Methanol-based industrial biotechnology: current status and future perspectives of methylotrophic bacteria.

Methanol is one of the building blocks in the chemical industry and can be synthesized either from petrochemical or renewable resources, such as biogas. Bioprocess technology with methylotrophic bacteria is well established, as illustrated by large-scale single-cell protein production in the past. During recent years, the first genomes of methylotrophs have been sequenced and significant progress in elucidating their metabolism has been made.

Staged lavage versus single high-volume lavage in the treatment of feculent/purulent peritonitis: a matched pair analysis.

Background: Spontaneous and iatrogenic secondary peritonitis remain to have a mortality of 10-30% and significant socioeconomic impact in survivors and especially non-survivors. Data on the most cost-effective treatment are lacking. We therefore studied outcome and resource utilization in a homogeneous cohort of patients with secondary fecal or purulent peritonitis undergoing surgery with source control and two different types of abdominal lavage.

Early excision of congenital melanocytic nevi under tumescent anesthesia and skin expansion by intracutaneous double butterfly sutures.

Background: Excision of large congenital melanocytic nevi is recommended because of the risk of malignant transformation and for aesthetic reasons. Our treatment concept includes 1) excision in one or more steps preferably before the age of one year, 2) skin expansion through high-tension wound closure with intracutaneous double butterfly sutures and 3) performing surgery with tumescent or general anesthesia.

Increased growth factor expression after hepatic and pancreatic resection.

Removal of the primary tumour is suggested to associate with an enhanced tumour growth of residual micrometastases. Recent data focus on growth factors that may be released in response to surgery-stimulating receptors of residual tumour cells. Vascular endothelial (VEGF) and hepatocyte growth factor (HGF) are potent inducers of angiogenesis.

Effects of ionizing irradiation and hydrostatic pressure on Escherichia coli O157:H7 inactivation, chemical composition, and sensory acceptability of ground beef patties.

A randomized complete block design with three replications was utilized to determine the effects of ionizing irradiation and hydrostatic pressure on the inactivation of Escherichia coli O157:H7, volatile composition, and consumer acceptability (n=155) of frozen ground beef patties. E-beam and X-ray irradiation (2kGy) inactivated E. coli O157:H7 below the limit of detection, while hydrostatic pressure treatment (300mPa for 5min at 4°C) did not inactivate this pathogen.

Relationships between sensory descriptors, consumer acceptability and volatile flavor compounds of American dry-cured ham.

The relationships between volatile flavor compounds, sensory descriptors and consumer acceptability were determined for eight commercial American dry-cured hams using external preference and flavor mapping. The majority of consumers preferred (p<0.05) hams that had more intense caramelized, smoky, savory and molasses aromas as well as more intense sweet and savory flavors.

Differential CXC receptor expression in colorectal carcinomas.

In this study, we aimed to assess the expression profile of chemokine receptors CXCR1-4 in inflammatory and malignant colorectal diseases and corresponding hepatic metastases of synchronous and metachronous origin to elucidate their role in colorectal cancer (CRC) progression and metastasis. Chemokine receptor expression was assessed by quantitative real-time PCR, immunohistochemistry (IHC) and Western blot analysis in resection specimens from patients with ulcerative colitis (UC, n = 25), colorectal adenomas (CRA, n = 8), different stages of CRC (n = 48) as well as colorectal liver metastases (CRLM) along with their corresponding primary colorectal tumours (n = 16). Indocyanine green disappearance rate is the most useful marker for liver resection.

Background/aims: Non-invasive measurement of indocyanine green plasma disappearance rate (PDR(ICG)) is supposed to be an accurate liver function parameter. However, its value compared to conventional markers like bilirubin and prothrombin time (PT) is unclear. The authors therefore prospectively determined PDR(ICG) and bilirubin and PT and recorded the clinical course after liver resection.

A young woman with recurrent vesicles on the lower lip: fixed drug eruption mimicking herpes simplex.

A 23-year-old woman presented with recurrent herpetiform vesicles of the lower lip, but all diagnostic measures for herpes virus infection including herpes viridae specific PCR were negative. Medical history revealed that she also had chronic recurrent vulvovaginal candidiasis, which had been treated with various regimes, including repetitive applications of fluconazole. Consequently, fluconazole-induced fixed drug eruption was suspected, but skin tests performed with fluconazole remained with-out response.

Enhanced ENA-78 and IL-8 expression in patients with malignant pancreatic diseases.

Methods: Protein expression profiles of the CXC chemokines growth-related oncogene alpha (GRO-alpha/CXCL1), epithelial cell-derived neutrophil-activating peptide-78 (ENA-78/CXCL5), granulocyte chemoattractant protein-2 (GCP-2/CXCL6), neutrophil-activating protein-2 (NAP-2/CXCL7), and interleukin-8 (IL-8/CXCL8) were assessed by enzyme-linked immunosorbent assay in pancreatic carcinoma, cancer of the papilla of Vater, pancreatic cystadenoma, and chronic pancreatitis specimens.

[Intraarterial thrombolysis in acute limb ischaemia: alternative treatment and basic principles of further interventions].

Introduction: Intraarterial thrombolysis of acute arterial occlusions in the lower limbs is an established therapeutic procedure. However, its value as a primary treatment is discussed controversially and it is mostly seen as a competing procedure to primary vascular surgical interventions.

Methods: From January 2001 to December 2004, we performed a total of 132 intraarterial thrombolysis procedures in 112 patients with acute or subacute ischaemia of the lower limbs.

Enrichment of phosphopeptides using biphasic immobilized metal affinity-reversed phase microcolumns.

Immobilized metal affinity chromatography (IMAC) based on Fe (3+) or Ga (3+) chelation is the most widely employed technique for the enrichment of phosphopeptides from biological samples prior to mass spectrometric analysis. An IMAC resin geared mainly toward phosphoprotein enrichment, Pro-Q Diamond, has been assessed for its utility in phosphopeptide isolation. Using both single phosphoprotein tryptic digests of beta-casein and ovalbumin and synthetic mixtures composed of tryptic digests of phosphorylated and nonphosphorylated protein standards, the selectivity and sensitivity of Pro-Q Diamond resin in an immobilized metal affinity-reversed phase microcolumn format were compared to an alternate titanium dioxide approach.

Standardizing experimental protocols.

Systems biology aims at understanding the behavior of biological networks by mathematical modeling based on experimental data. However, frequently experimental data is derived from poorly defined cellular systems, the procedures of data generation are insufficiently documented and data processing is arbitrary. For the advancement of systems biology, standardization at multiple levels is essential.

Surgery of liver metastasis in gynecological cancer - indication and results.

Background: Liver surgery for patients with liver metastases from gynecological malignancies, an indicator of advanced cancer disease, has remained unclear in the literature. We therefore analyzed the potential survival benefit of patients with surgically resectable compared to unresectable liver metastases.

Patients And Methods: 43 patients who underwent surgery for liver metastases from gynecological cancers were included in our retrospective observational analysis.
