Extensor Retinaculum Syndrome of the Ankle: An Adult Case Series.

Background: Extensor retinaculum syndrome (ERS) is a relatively rarely diagnosed compartment syndrome-like entity caused by elevated pressures in the tissues deep to the superior extensor retinaculum (SER). ERS is identified as out-of-proportion anterior ankle pain, pain with passive toe plantarflexion, elevated SER pressures (>40 mm Hg), and ultimately toe extension weakness and first web space numbness. Although previously described in a pediatric population, this case series is the first to our knowledge in an adult population.

Organic fertilization and mycorrhization can increase the phytoremediation of copper-contaminated soils. The time of vermicomposting alters the properties of vermicompost, which can affect copper's availability and uptake. Therefore, this study sought to evaluate the effect of different organic fertilizers and mycorrhization on copper-contaminated soil phytoremediation.

A Sustainable Model for Preventive Services in Rural Counties: The Healthier Together Study.

Objective: The Healthier Together study aimed to implement and evaluate a sustainable, rural community-based patient outreach model for preventive care provided through primary care practices located in 3 rural counties in Oklahoma. Community-based wellness coordinators (WCs) working with primary care practitioners, county health departments, local hospitals, and health information exchange (HIE) networks helped residents receive high-priority evidence-based preventive services.

Methods: The WCs used a wellness registry connected to electronic medical records and HIEs and called patients at the county level, based on primary care practitioner-preferred protocols.

Can compassion focused therapy enhance dual recovery for veterans?

In our clinical work with veterans experiencing serious mental illness and substance use disorders, we noticed specific challenges not commonly addressed with traditional dual recovery group therapy-including having overactive threat systems and feelings of shame and self-criticism. For a program development project at our Veterans Affairs Psychosocial Rehabilitation and Recovery Center, we designed and implemented a group therapy program based on Compassion Focused Therapy (CFT), a therapeutic model that directly addresses the aforementioned challenges (Gilbert, 2009b). Results from pre- and post-intervention measures suggested veterans gained compassion and mindfulness skills and experienced reduced depressive symptoms.

Characterization of intestinal Escherichia coli isolated from calves with diarrhea due to rotavirus and coronavirus.

Purpose: To address more information about changes in commensal Escherichia coli during virus intestinal infection, we characterized 30 faecal E. coli isolates from calves (21 to 60 days old) with diarrhea due to rotavirus and coronavirus, which received, before diagnosis, tetracycline, gentamicin and enrofloxacin drugs.

Methodology: Clermont's phylogenetic classification; presence of genes for curli, cellulose, fimbriae (F4, F5, F6, F18, F41); and antimicrobial susceptibility were used to characterize the isolates.

Congestive heart failure (CHF) is a major cause of morbidity and mortality. Early diagnosis of CHF in patients presenting to the emergency department (ED) with undifferentiated dyspnea would allow clinicians to begin appropriate treatment more promptly. Current guidelines recommend B-type natriuretic peptide (BNP) levels for more accurate diagnosis of CHF in dyspneic patients.

Synthesis of Breathing Metallopolymer Hollow Spheres for Redox-Controlled Release.

A convenient synthetic approach for the preparation of uniform metallopolymer-containing hollow spheres based on 2-(methacryloyloxy)ethyl ferrocenecarboxylate (FcMA) as monomer by sequential starved feed emulsion polymerization is described. Core/shell particles consisting of a noncrosslinked poly(methyl methacrylate) core and a slightly crosslinked ferrocene-containing shell allows for the simple dissolution of core material and, thus, monodisperse metallopolymer hollow spheres are obtained. Growth of tropical tree species and absorption of copper in soil artificially contaminated.

Reclamation of copper contaminated sites using forest species may be an efficient alternative to reduce the negative impact. The aim of this study was to quantify the growth and evaluate the quality of seedlings of native species at different doses of copper in the soil. The experimental design was completely randomized, with seven replications in a factorial arrangement (3×9), using three indigenous species of plants (Anadenanthera macrocarpa, Mimosa scabrella and Apuleia leiocarpa) and nine doses of copper in the soil (0, 60, 120, 180, 240, 300, 360, 420 and 480 mg kg-1).

Metallopolymer-Based Shape Anisotropic Nanoparticles.

The formation of nanostructured shape anisotropic nanoparticles from poly(ferrocenylsilane)--poly(2-vinylpyridine) (PFS--P2VP) block copolymers is presented. Ellipsoidal particles with an axially stacked lamellar structure and nanosheets with a hexagonal structure of PFS cylinders are obtained under neutral wetting conditions through the use of a mixed surfactant system during self-assembly. In contrast to traditional systems, the resulting particle structure is strongly influenced by crystallization of the PFS domains under colloidal confinement with lamella-forming PFS--P2VP block copolymers leading to cylindrical morphologies.

Single-source magnetic nanorattles by using convenient emulsion polymerization protocols.

A novel strategy to achieve easily scalable magneto-responsive nanoceramics with core/shell and nanorattle-type or yolk/shell architectures based on a ferrocene-containing polymer precursor is described. Monodisperse nanorattle-type magnetic particles are obtained by using convenient semicontinuous emulsion polymerization and Stöber process protocols followed by thermal treatment. The particles are characterized by TGA, TEM, WAXS, DLS, XPS, and Raman spectroscopy.

HIE-i-health information exchange with intelligence.

This article demonstrates the development and pilot testing of an innovative approach to implement health information exchange with intelligence (HIE-i) in primary care settings. Records of 346 patients were studied in 6 primary care practices. Clinical workflows were evaluated by time motion studies and observations.

Improving colorectal cancer screening in family medicine: an Oklahoma Physicians Resource/Research Network (OKPRN) study.

Purpose: The purpose of this study was to describe colorectal cancer screening (CRCS) practices across a variety of primary care clinics and identify the methods used by primary care physicians (PCPs) with higher rates of CRCS ("exemplars").

Methods: Physician questionnaires, structured interviews, medical record abstractions, and practice observations were conducted for 48 PCPs in 25 practices within a regional practice-based research network followed by secondary in-depth interviews to further investigate the practices of PCPs in the top quartile of CRCS rates ("exemplars").

Results: We abstracted 3596 medical records (mean of 75 records per PCP).

A pharmacist visit improves diabetes standards in a patient-centered medical home (PCMH).

Many patients with diabetes do not receive recommended standards of care. Diabetes patients were seen by a pharmacist in a diabetes assessment service (DAS) 1 week prior to a physician appointment to complete diabetes standards. Completion rates of American Diabetes Association (ADA) standards were compared between patients of 5 physicians offered the DAS intervention and a concurrent cohort of randomly selected patients of nonparticipating physicians.

Treatment of large segmental bone defects with reamer-irrigator-aspirator bone graft: technique and case series.

Treatment of large segmental defects using conventional autogenous iliac crest bone graft can be limited by volume of cancellous bone and donor site morbidity. The reamer-irrigator-aspirator (RIA) technique allows access to a large volume of cancellous bone graft containing growth factors with potency equal to or greater than autograft material from the iliac crest. The purpose of this study was to evaluate the effectiveness of RIA-harvested autogenous bone graft for treating large segmental defects of long bones.

The gallium(I) derivative [Ga({N(dipp)CMe}(2)CH)] (1; dipp = 2,6-diisopropylphenyl) undergoes facile oxidative addition reactions with various element-hydrogen bonds including N-H, P-H, O-H, Sn-H, and H-H bonds. This was demonstrated by its reaction with triphenyltin hydride, ethanol, water, diethylamine, diphenylphosphane, and dihydrogen. All products were characterized by means of single-crystal X-ray structure determination, NMR spectroscopy, IR spectroscopy, and mass spectrometry.

Gas gangrene or clostridial myonecrosis is a rare, life threatening infection of the muscle that is typically associated with recent surgery or trauma. It usually affects older individuals who suffer from vascular insufficiency or who are immunocompromised. The typical pathogen is Clostridium perfringens.

The frequency of sleep disruption and the degree to which insomnia significantly affects daytime function determine the need for evaluation and treatment. Physicians may initiate treatment of insomnia at an initial visit; for patients with a clear acute stressor such as grief, no further evaluation may be indicated.
