Article Synopsis
  • The study explores boiling histotripsy (BH) for non-invasive kidney tumor ablation, focusing on different BH dose metrics in porcine and canine kidney models.
  • Various pulsing protocols were tested to assess lesion formation, revealing that in vivo kidney tissue required lower BH doses compared to ex vivo tissue for similar damage.
  • Results indicated tissue stiffness and damage differed between kidney compartments, with the renal cortex needing fewer pulses for effective treatment, while the medulla and renal sinus showed higher resistance to BH dosing.

Article Synopsis
  • Small cell bladder cancer (SCBC) is a rare and aggressive type of cancer, prompting a study to compare outcomes of two treatment methods: cystectomy with neoadjuvant chemotherapy (NAC) and concurrent chemoradiotherapy (CCRT).
  • The study examined data from an institutional database and SEER-Medicare, identifying 53 and 1166 patients, respectively, to evaluate overall survival (OS) and found that NAC followed by cystectomy resulted in a median OS of around 45-46 months compared to lower OS of 23-26 months with CCRT.
  • Although NAC + cystectomy correlated with a potential 30% reduction in mortality compared to CCRT, results were not statistically significant, highlighting

Histotripsy is a noninvasive focused ultrasound therapy that mechanically fractionates tissue to create well-defined lesions. In a previous clinical pilot trial to treat benign prostatic hyperplasia (BPH), histotripsy did not result in consistent objective improvements in symptoms, potentially because of the fibrotic and mechanically tough nature of this tissue. In this study, we aimed to identify the dosage required to homogenize BPH tissue by different histotripsy modalities, including boiling histotripsy (BH) and cavitation histotripsy (CH).

Airborne radioactivity from fossil fuel production systems is poorly characterized, but a recent study showed elevated ambient levels with proximity to oil and gas production wells. Here, we report year-long, high temporal resolution monitoring results of airborne alpha radioactivity from both radon gas and radon progeny attached to particulates immediately northeast of an oil refinery in Commerce City, Colorado, USA, in an environmental justice community of concern. Gas and particle-associated radioactivity contributed nearly evenly to the total alpha radioactivity.

Objective: To evaluate simulated parastomal herniation forces in in vitro abdominal fascial models. Our group previously illustrated how incision type may play a consequential role in bowel herniation force generated across an incision using several abdominal fascia models. We sought to (1) Confirm findings in fresh human tissue, (2) Assess correlation between herniation force and incision size, and (3) Determine whether incision type impacts drainage in a simulated ex vivo ileal conduit.

High-intensity focused ultrasound (HIFU) applications for thermal or mechanical ablation of renal tumors often encounter challenges due to significant beam aberration and refraction caused by oblique beam incidence, inhomogeneous tissue layers, and presence of gas and bones within the beam. These losses can be significantly mitigated through sonication geometry planning, patient positioning, and aberration correction using multielement phased arrays. Here, a sonication planning algorithm is introduced, which uses the simulations to select the optimal transducer position and evaluate the effect of aberrations and acoustic field quality at the target region after aberration correction.

Introduction: Cisplatin-based neoadjuvant chemotherapy (NAC) followed by cystectomy is the standard for muscle-invasive bladder cancer (MIBC), however, NAC confers only a small survival benefit and new strategies are needed to increase its efficacy. Pre-clinical data suggest that in response to DNA damage the tumor microenvironment (TME) adopts a paracrine secretory phenotype dependent on mTOR signaling which may provide an escape mechanism for tumor resistance, thus offering an opportunity to increase NAC effectiveness with mTOR blockade.

Patients & Methods: We conducted a phase I/II clinical trial to assess the safety and efficacy of gemcitabine-cisplatin-rapamycin combination.

Purpose: Active surveillance is a safe and effective strategy for men with lower-risk prostate cancer who want to avoid local therapy; however, many patients on active surveillance progress to active treatment (eg, prostatectomy or radiation). We hypothesized that apalutamide would decrease active surveillance attrition rates through downstaging low-grade tumors.

Materials And Methods: This was an open-label, single-arm, phase II study testing 90 days of oral apalutamide 240 mg daily in men with low- to intermediate-risk prostate cancer on active surveillance.

Article Synopsis
  • A study looked at a surgery called cytoreductive nephrectomy (CN) for patients with advanced kidney cancer who received immunotherapy and wanted to see how it affects their survival.
  • The research included 367 patients, and those who had CN along with immunotherapy lived longer (about 56.3 months) compared to those who only had immunotherapy (about 19.1 months).
  • The results suggest that CN can help patients with metastatic kidney cancer live longer when combined with immunotherapy, so doctors might consider it for some patients.

Boiling histotripsy (BH) is a focused ultrasound technology that uses millisecond-long pulses with shock fronts to induce mechanical tissue ablation. The pulsing scheme and mechanisms of BH differ from those of cavitation cloud histotripsy, which was previously developed for benign prostatic hyperplasia. The goal of the work described here was to evaluate the feasibility of using BH to ablate fresh ex vivo human prostate tissue as a proof of principle for developing BH for prostate applications.
