Association of endothelial dysfunction with sarcopenia and muscle function in a relatively young cohort of kidney transplant recipients.

Background: Sarcopenia and endothelial dysfunction are both common among kidney transplant (KT) recipients. We aimed to evaluate the association between endothelial dysfunction and sarcopenia, as well as its individual components.

Methods: Vascular reactivity index (VRI), skeletal muscle index (SMI = skeletal muscle mass/height), handgrip strength (HGS), and 6-meter usual gait speed (GS) were measured in 95 KT recipients.

Synthesis, characterization and biological properties of cobalt(II) complexes of 1,10-phenanthroline and maltol.

The synthesis and characterization of two cobalt(II) complexes, Co(phen)(ma)Cl 1 and Co(ma)(2)(phen) 2, (phen=1,10-phenanthroline, ma(-)=maltolate or 2-methyl-4-oxo-4H-pyran-3-olate) are reported herein. The complexes have been characterized by FTIR, CHN analysis, fluorescence spectroscopy, UV-visible spectroscopy, conductivity measurement and X-ray crystallography. The number of chelated maltolate ligands seems to influence their DNA recognition, topoisomerase I inhibition and antiproliferative properties.

Acute grief with delirium in an elderly: holistic care.

Delirium in the elderly is a challenging and under-recognized problem in the community. Early detection and management improves outcomes and quality of life for the elders with delirium at home.1 Family physicians (FP) play a key role in the assessments, early identification, and management of delirium and in the support and education of patients and their family caregivers.

An 80-year-old woman with left shoulder pain.

This case history illustrates the real life experience and dilemma of an 80-year-old woman in pursuit of medical care for her left shoulder pain. Points for discussion range from clinical features of Pancoast tumor, importance of pain management, good principles of Family Medicine and Palliative care to ethical issues of conspiracy of silence, limited treatment plan and palliative versus curative radiotherapy treatment without a known biopsy report. This paper provides opportunity for analysis of a real complex clinical situation, application of medical knowledge to problem solving in clinical practice and relevant topics for discussions.

Academic mobbing: hidden health hazard at workplace.

Academic mobbing is a non-violent, sophisticated, 'ganging up' behaviour adopted by academicians to "wear and tear" a colleague down emotionally through unjustified accusation, humiliation, general harassment and emotional abuse. These are directed at the target under a veil of lies and justifications so that they are "hidden" to others and difficult to prove. Bullies use mobbing activities to hide their own weaknesses and incompetence.

Impact of cancer on psychosexuality: cultural perspectives of Asian women.

Cancers and related treatments have devastating effects on psychosexual life of patients. This study helps us to understand the cultural perspectives of 50 Asian women diagnosed with cancer. Median age was 50+ years.

Masquerades of cluster headache.

Many cluster headache (CH) patients waited several years to be accurately diagnosed because their symptoms are often mistaken for sinusitis or a dental disorder.1 Patients have also been mistakenly diagnosed as analgesic drug abusers or suffering from a psychiatric illness. This case report illustrates how a young lady was diagnosed to have cluster headache after several years of consultations with ophthalmologists for eye swelling and redness.

The role of general practitioners in home oxygen therapy.

Recommendation of oxygen therapy must include clear indication and benefits of its use, appropriate prescription, vigilant monitoring and appropriate methods of delivery. Home oxygen therapy is expensive, inconvenient and cumbersome; it should be recommended only if benefits outweigh the disadvantages and adverse effects of oxygen. GPs play an important supportive and supervisory role in the use of long-term oxygen therapy (LTOT) to improve mortality of patients with chronic hypoxaemia.

Antiplasmodial activity of ferrocenyl chalcones: investigations into the role of ferrocene.

A series of ferrocenyl chalcones were synthesized and evaluated in vitro against Plasmodium falciparum (K1) in a [3H] hypoxanthine uptake assay. Appropriate size, electronic, lipophilic and electrochemical parameters were determined for QSAR analysis. The results showed that the location of ferrocene influenced the ease of oxidation of Fe2+ in ferrocene and the polarity of the carbonyl linkage.

Electrochemically generated super-hydrophilic surfaces.

The control of surface wettability of indium tin oxide (In2O3-SnO2) coated glass surfaces is studied. We report here the first observation of purely electrochemical generation of super-hydrophilic surfaces at In2O3-SnO2 electrodes in the absence of an ultraviolet illumination source. This is achieved by the application of high anodic potentials for a prolonged period.

Palliative care: experience with 156 cases in a general hospital.

The concept of Palliative Care is still quite new in Malaysia and there is a need to promote the awareness of its importance for patients with incurable and advanced illnesses, not only to the public but also to the nursing and medical professionals. These patients especially the poor ones very often have no one to turn to; they cannot afford to seek treatment from the private hospitals, they are turned away from acute General Hospitals and are told that there is nothing more to be done because their illnesses are no longer curable, they cannot pay for GPs to come to their homes, and there is difficulty in purchasing opiate drugs for pain relief. This is a retrospective observational study of the palliative care services we try to provide to those few patients referred to us.

Speciation analysis of tellurium by solid-phase extraction in the presence of ammonium pyrrolidine dithiocarbamate and inductively coupled plasma mass spectrometry.

Under acidic conditions tellurium(IV) formed a complex with ammonium pyrrolidine dithiocarbamate (APDC). The tellurium(IV) complex was completely retained on a non-polar Isolute silica-based octadecyl (C(18)) sorbent-containing solid-phase extraction (SPE) cartridge, while the uncomplexed Te(VI) passed through the cartridge and remained as a free species in the solution. Only partial Te(IV) was retained on the SPE cartridge for samples without addition of APDC.

Synthetic, structural, electrochemical, and theoretical studies of heterometallic aggregates with a [Pt(2)(mu-S)(2)M] core (M = Hg, Au).

Novel electroactive multimetallic compounds based on the [Pt(2)(mu(2)-S)(2)M] core, viz. [Pt(2)(PPh(3))(4)(mu(3)-S)(2)HgFc]PF(6) (1) [Fc = (eta(5)-C(5)H(4))Fe(eta(5)-C(5)H(5))] and [Pt(2)(PPh(3))(4)(mu(3)-S)(2)Hg(2)Fc'](PF(6))(2) (2) [Fc' = Fe(eta(5)-C(5)H(4))(2)], have been synthesized under the guide of electrospray mass spectrometry. The electrochemistry of these ferrocene funtionalized compounds together with the reported [Pt(2)(PPh(3))(4)(mu(3)-S)(2)HgPPh(3)](PF(6))(2) (3), [Pt(2)(PPh(3))(4)(mu(2)-S)(mu(3)-S)HgPh]PF(6) (4), and [Pt(2)(PPh(3))(4)(mu(2)-S)(mu(3)-S)AuPPh(3)]PF(6) (5) have been investigated using cyclic voltammetry and DFT calculations.

Studies of sol-gel ceramic film incorporating methylene blue on glassy carbon: an electrocatalytic system for the simultaneous determination of ascorbic and uric acids.

In this work, we investigated the immobilization of methylene blue in a methyltrimethoxysilane sol-gel ceramic film on a glassy carbon electrode. Up to a certain saturation level, under our conditions, it was found that the methylene blue was tightly held and did not leach out into aqueous solutions, even with continuous immersion for up to 1 month. The electrochemical behavior of the immobilized methylene blue was then studied.

Antimony speciation by inductively coupled plasma mass spectrometry using solid phase extraction cartridges.

A novel and simple method for inorganic antimony speciation is described based on selective solid phase extraction (SPE) separation of antimony(III) and highly sensitive inductively coupled plasma mass spectrometric (ICP-MS) detection of total antimony and antimony(V) in the aqueous phase of the sample. Non-polar SPE cartridges, such as the Isolute silica-based octyl (C8) sorbent-containing cartridge, selectively retained the Sb(III) complex with ammonium pyrrolidine dithiocarbamate (APDC), while the uncomplexed Sb(V) remained as a free species in the solution and passed through the cartridge. The Sb(III) concentration was calculated as the difference between total antimony and Sb(V) concentrations.

Optimization of the simultaneous batch determinations of Bi(III), Hg(II) and Cu(II) at an epoxy-graphite electrode bulk modified with 2-mercaptobenzothiazole.

An epoxy-graphite tube composite electrode bulk modified with 2-mercaptobenzothiazole was investigated for the simultaneous, batch determinations of Bi(III), Hg(II) and Cu(II) by differential-pulse anodic stripping voltammetry after open circuit preconcentration of the three metal ions. Systematic optimization was studied with respect to resolution and sensitivities of the differential-pulse peaks. The resolution between peaks was quantified and employed in a window diagram approach to choose the best conditions.

Separation of selected PAHs by using high performance capillary electrophoresis with modifiers.

The effect of varying SDS concentration in a micellar electrokinetic capillary chromatographic system (MECC) is investigated. In addition, the introduction of γ-cyclodextrin as modifier to the electrophoretic medium, containing SDS and a phosphate-borate buffer, is shown to give satisfactory separation of the seven selected PAHs. The separation efficiency is increased with the addition of γ-cyclodextrin.
