We present a case of haemobilia as a primary presentation for underlying cholangiocarcinoma. A man in his 50s initially presented to emergency with Quincke's triad, RUQ pain, jaundice and UGI bleeding. The initial diagnosis of haemobilia was made on endoscopic retrograde cholangiopancreatography (ERCP) on primary presentation, but the presence of blood and the recurrent clot obstruction of the biliary tract made the underlying diagnosis extremely difficult, resulting in the patient having 4 ERCP, 1 spyglass and multiple CTs and magnetic resonance cholangiopancreatography.

Pancreatic neuroendocrine tumour resection in circumportal pancreas: a rare anatomical anomaly with important surgical implications.

Various congenital anomalies of the pancreas have been reported due to its complex embryological development involving the fusion of two separate buds. Circumportal pancreas is a rare anatomical anomaly where the pancreatic head and uncinate process fuse abnormally with the pancreatic body, encasing the portal vein and/or superior mesenteric vein completely. This anomaly poses several challenges to hepatobiliary surgeons, as the encasement of the portal vein by the abnormal pancreatic tissue makes an additional parenchymal transection necessary.

This study aimed to investigate the association between polyvictimization and offline and online sexual harassment and whether the association might be mediated by internalizing problems, low school satisfaction, and dropping out of school. The analytic sample was derived from the Children and Youth Rights Survey in South Korea. It consisted of 6353 adolescents' responses to the sexual harassment question and a set of other questions in the middle and high school questionnaires.

Evaluation of a Home-Based, Nurse Practitioner-led Advanced Illness Care Program.

Objectives: In the United States, people with serious illness often experience gaps and discontinuity in care. Gaps are frequently exacerbated by limited mobility, need for social support, and challenges managing multiple comorbidities. The Advanced Illness Care (AIC) Program provides nurse practitioner-led, home-based care for people with serious or complex chronic illnesses that specifically targets palliative care needs and coordinates with patients' primary care and specialty health care providers.

Laterality and severity of nasal obstruction does not correlate between physicians and patients, nor among physicians.

Background: Nasal obstruction is a common patient complaint and has a variety of etiologies, and a specific anatomical abnormality can often be found within the nasal cavity on physical examination. In practice, this observed pathology does not always correlate with the laterality, severity, and exact intranasal site of the patients' perceived obstruction.

Objectives: We seek to answer the following questions: 1) Does a physician's evaluation of nasal obstruction correlate with subjective patient complaints? 2) Is there reasonable correlation between physicians of similar training in the routine evaluation of nasal obstruction?

Methods: First, we asked patients presenting to the otolaryngology clinic with a primary complaint of nasal obstruction to fill out a modified NOSE survey.

Salvage Treatment Using Anti-PD-1/CTLA-4 Immunotherapy After Failure of Neoadjuvant Chemotherapy in Microsatellite Instable Gastroesophageal Carcinoma.

Perioperative chemotherapy is standard treatment for patients with early high-risk gastroesophageal adenocarcinoma independent of molecular subtype. Approximately 8% of gastroesophageal cancers have a microsatellite instable phenotype (MSI-H), and retrospective analyses of neoadjuvant/adjuvant chemotherapy trials suggests no survival benefit in this patient population compared with surgery alone. HUWE1 mutations in Juberg-Marsidi and Brooks syndromes: the results of an X-chromosome exome sequencing study.

Background: X linked intellectual disability (XLID) syndromes account for a substantial number of males with ID. Much progress has been made in identifying the genetic cause in many of the syndromes described 20-40 years ago. Next generation sequencing (NGS) has contributed to the rapid discovery of XLID genes and identifying novel mutations in known XLID genes for many of these syndromes.

Reversible ligand binding reactions: Why do biochemistry students have trouble connecting the dots?

Adaptive chemical behavior is essential for an organism's function and survival, and it is no surprise that biological systems are capable of responding both rapidly and selectively to chemical changes in the environment. To elucidate an organism's biochemistry, its chemical reactions need to be characterized in ways that reflect the normal physiology in vivo. This is a challenging experimental problem because biological systems are inherently complex with myriads of interlinked chemical networks orchestrating processes that are mostly irreversible in nature.

Muslims in prison: a case study from Ohio state prisons.

The present study explores core issues related to the understudied population of Muslim inmates. Mail questionnaires were sent to the full-time chaplains employed by religious services in thirty Ohio state male prisons. The survey examines: (1) characteristics of Muslim inmates, (2) patterns of identification with Islam, (3) religious behavior inside the prisons, and (4) relations between conversion to Islam and crime committed.

The influence of ethnicity, social class, and context on judgments about U.S. women.

In 2 studies, the authors investigated impression formation as influenced by category-based stereotypes associated with ethnicity and social class. The participants in Study I made judgments about 1 target woman, described as interested in running for office in the Parent Teacher Organization (PTO) of her children's school. The hypothetical woman was presented to the respondents along with her photograph and information about her ethnic background (Anglo-Saxon, Latina, or Jewish) and occupation (middle class or working class).

Aortocaval fistula and contained rupture of an abdominal aortic aneurysm presenting with pelvic venous congestion.

This case report describes pelvic venous congestion without associated high-output cardiac failure as an unusual presentation of an aortocaval fistula as a complication of a contained rupture of an abdominal aortic aneurysm. Options for caval control and management of perioperative problems unique to this situation are discussed.

Legislative and humanitarian impetus for development of alcohol and other drug policy at an Australian university.

The rationale for comprehensive alcohol and other drug policy at a tertiary institution is presented here as a health promotion strategy for educating University policy makers. Particular emphasis is placed on a discussion of humanitarian issues, that is, the right to a healthy environment, and the potential for legal liability in an environment where alcohol is readily available and other drugs are present. There is little attention paid to drug use related legal issues in the University, but at the same time drawing the attention of policy makers to the organisation's vulnerability to lawsuits has been reported to be a motivating force for adoption of drug use policy.

Effects of continuous infusion of intravenous nitroglycerin on methemoglobin levels.

The effect of continuous infusion of intravenous nitroglycerin (NTG) on methemoglobin levels in 24 coronary care unit patients was studied. Fifteen patients were admitted with acute myocardial infarction, 5 patients with angina pectoris, 1 patient with congestive heart failure, 2 with chest pain of unknown origin and 1 with chest pain of musculoskeletal origin. Intravenous NTG therapy was initiated at a 5 micrograms/min dose and titrated at 5-micrograms increases until relief of symptoms or until the maximal dose tolerated by each patient.

Long-term neuropsychological deficits after traumatic head injury with comatosis.

Studied neuropsychological deficits in a population who had sustained traumatic head injuries of the concussive type. Experimental Ss were 48 adults who had been comatose and hospitalized during the 3-year period that preceded the study. Control Ss (N = 16) had been hospitalized due to other types of injuries and had not been comatose.

Neurologic sequelae of Rocky Mountain spotted fever.

Forty-two children who had Rocky Mountain spotted fever were seen for neurologic and psychologic examination at follow-up. Lower salary costs and effective provision of services to specific community groups are counted among the advantages of the use of mental health workers in community programs. In a cost-benefit analysis of the value of health manpower, salaries and service provision are only two of several factors, yet the circumstances of funding, training, and role diffusion that surround the use of the mental health worker have precluded true cost-benefit analysis. The authors believe that the mental health worker program of the Orange County, California, community mental health program is developed and stable encough that the initial steps of an economic analysis are possible.

Self-destructive behavior patterns in male and female drug abusers.

One hundred and fourteen male and female drug abusers were interviewed regarding their histories of self-destructive behaviors. The data were analyzed to determine if, and identify what, differences existed between males and females with respect to types and extent of self-destructiveness. The analyzed data indicate that male and female drug abusers present very different self-destructive portraits.
