We report an autopsy case with progressive muscular atrophy of 6 years' duration. The patient was a Japanese woman without any hereditary predisposition to neuropathy. She developed muscle weakness of the upper extremities at 66 years of age, followed by muscle weakness of the lower extremities, fasciculation, and atrophy of the tongue.

Methods: A cross-sectional questionnaire survey. Patients Outpatients who visited us from March 1 to May 30, 2012, were asked to complete a questionnaire survey regarding SAPPHIRE and m-SAPPHIRE.
A 55-year-old man complained of intermittent piercing pain on the right side of his throat. In a visit to our hospital, he was diagnosed with glossopharyngeal neuralgia and was treated with carbamazepine. But he obtained no pain relief and had drug-induced hypersensitivity syndrome due to carbamazepine on admission.
