We report the case of a 69-year-old female who underwent a Baerveldt implant placement for severe-stage primary open-angle glaucoma and developed a bacterial infection of the conjunctiva and abscess of the scleral patch graft with subsequent tube exposure. The infection was identified 3 weeks postoperatively and a topical antibiotic was immediately initiated. A concurrent systemic staphylococcal infection was discovered by an outside physician and oral cephalexin was initiated.

Article Synopsis
  • The study investigates a unique case of drug-induced angle-closure glaucoma linked to aripiprazole, an atypical antipsychotic used for treating depression.
  • A 45-year-old woman developed acute angle closure after starting aripiprazole, showing elevated intraocular pressure and specific eye configurations; she underwent procedures to lower the pressure and manage her condition.
  • The findings suggest that aripiprazole may cause angle-closure due to its serotonin effects on the eye, and if necessary for depression treatment, the medication should be approached cautiously with monitoring.

Though heterophile antibody tests are often ordered to confirm diagnosis of IM, this test has a high false-negative rate early in the course of the disease.
