Although silent atrial fibrillation (AF) accounts for a significant proportion of patients with AF, asymptomatic patients have been excluded from AF ablation trials. This population presents unique challenges to disease management. Recent evidence suggests that patients with asymptomatic AF may have a different risk profile and even worse long-term outcomes compared to patients with symptomatic AF.

Authors evaluated the effects of selective adrenergic blockade by means of doxazosin on blood pressure in 48 patients operated on for pheochromocytoma by a multicenter retrospective study. Age, tumor size, surgical approach, and operative time were analyzed as predictive factors of intraoperative hypertensive crises. Forty-eight patients underwent adrenalectomy--four open surgery and 44 laparoscopic surgery--for pheochromocytoma of adrenal glands from 1998 to 2008 after preoperative administration of doxazosin.

Chordoma of thoracic vertebras is a very rare locally invasive neoplasm with low grade malignancy arising from embryonic notochordal remnants. Radical surgery remains the cornerstone of the treatment. We describe a case of multifocal T1-T2 chordoma, without bone and disc involvement, incidentally misdiagnosed as a paraganglioma, occurring in a 47-year-old male asymptomatic patient.

The aim of the study was to evaluate swallowing using a reproducible objective methodology and to seek preoperative factors that could influence swallowing outcomes in patients operated on for partial laryngectomy. Twenty-four patients who underwent partial frontolateral laryngectomy with epiglottic reconstruction for T1bN0 or T2N0 glottic carcinoma between 2008 and 2012 were retrospectively evaluated. Using fiberoptic endoscopic and videofluoroscopic evaluation, early (15 days postoperatively) and late (2 months postoperatively) scores were obtained for all patients to quantify their swallowing skills.

Aim: Cervical lymph node micrometastases are observed in up to 90% of papillary thyroid cancers (PTC), showing that lymph nodal involvement is very common. Nevertheless, during the last years, the role of lymph node dissection in the treatment of PTC has been controversial and, at present, the best indications to the routine or therapeutic neck dissection remain subject of research. In order to better analyze the current role of lymph node dissection in the surgical treatment of PTC, an analysis of the most recent literature data was performed.

Article Synopsis
  • The study aimed to investigate the reliability of silicone-polyurethane copolymer (Optim™) lead insulation in implantable cardioverter-defibrillator (ICD) patients due to concerns about potential defects.
  • Between 2007 and 2011, 234 patients with 413 Optim-coated leads were evaluated using fluoroscopy and electrical assessments, revealing seven lead failures and no visible structural defects after follow-up.
  • Overall, the results indicated a low lead failure rate and normal fluoroscopic appearances, providing initial evidence of the Optim lead insulation's reliability over five years in a large patient cohort.

Purpose: A new four-pole connector system (DF-4) for transvenous high-voltage implantable cardioverter defibrillators (ICD) is currently available in clinical practice. However, no clinical data demonstrating the safety and effectiveness of this complex electromechanical design is available. This study aims to test the safety and effectiveness of this newly designed system compared to the conventional DF-1 leads.

Introduction: Temporal bone fracture is a common complication of high-energy cranial trauma. Labyrinth involvement is rare, but there is a risk of perilymphatic rupture that is often underestimated on initial clinical examination due to the predominance of neurological and/or somatic symptoms.

Case Report: A patient presented with overlooked perilymphatic fistula, decompensated by hyperpressure effort due to poorly adapted management.

Epidemiological studies have demonstrated that the Mediterranean diet, which is rich in resveratrol, is associated with a significantly reduced risk of cardiovascular disease. However, the molecular mechanisms that underlie the beneficial effects of resveratrol on cardiovascular function remain incompletely understood. Therefore, we set out to identify the molecular target(s) mediating the protective action of resveratrol on vascular function.

Article Synopsis
  • Cardiac resynchronization therapy (CRT) has shown significant improvement in heart failure patients, who often experience endothelial dysfunction (ED) characterized by impaired flow-mediated vasodilation (FMD).
  • In a study of 57 heart failure patients undergoing CRT with defibrillators, a notable 64.9% were found to have ED, but after 12 months of CRT treatment, FMD increased significantly from 4.1% to 8.8%.
  • Although ED does not independently predict CRT response, it can serve as a useful marker for assessing CRT effectiveness, particularly in patients who struggle with the 6-minute walk test.

In the last years, remote monitoring systems have been continuously updated and implemented by new algorithms to ensure proper and highly individualized management of patients with pacemakers or implantable defibrillators. The clinical history of patients with implantable devices is frequently complicated by the presence of atrial fibrillation and heart failure, often coexisting in the same subject, which are frequently responsible for major events. These patients, therefore, are those who benefit most from the remote monitoring of implantable devices.

Mapping of epitopes recognized by functional monoclonal antibodies (mAbs) is essential for understanding the nature of immune responses and designing improved vaccines, therapeutics, and diagnostics. In recent years, identification of B-cell epitopes targeted by neutralizing antibodies has facilitated the design of peptide-based vaccines against highly variable pathogens like HIV, respiratory syncytial virus, and Helicobacter pylori; however, none of these products has yet progressed into clinical stages. Linear epitopes identified by conventional mapping techniques only partially reflect the immunogenic properties of the epitope in its natural conformation, thus limiting the success of this approach.

Total thyroidectomy (TT) is the standard of care for differentiated thyroid cancer (DTC), but still there is no consensus about the role of routine use of prophylactic central lymph node dissection. The aim of this study was to analyze our results of TT without prophylactic central lymphadenectomy in the treatment of DTC. Clinical records, between January 1998 and December 2005, of 221 patients undergoing TT, without prophylactic central lymph node dissection, were retrospectively evaluated.

Background: Minimally invasive adrenalectomy for pheochromocytoma (PCC) is a complex surgical procedure especially because of the haemodynamic instability due to the excessive secretion of cathecolamines, which may result in a considerable risk of conversion and complications.

Aim: By a multicentric retrospective study, the authors reported the results of laparoscopic adrenalectomies in patients operated for adrenal pheochromocytomas with the aim of investigating the role of the laparoscopic approach in managing adrenal disease.

Methods: Sixty patients, preoperatively treated with a selective alpha1-blocker, underwent 61 laparoscopic adrenalectomies for PCC smaller or larger than 6 cm - group A (≤6 cm), group B (>6 cm).

The worldwide use of both magnetic resonance imaging (MRI) and pacing devices has vastly increased in recent years and an important number of implanted patients likely will need an MRI over the course of the lifetime of their device. Although some studies have demonstrated that, given appropriate precautions, MRI can be safely performed in patients with selected implantable pacemakers, MRI in pacemaker patients is still contraindicated. Recently, new pacing systems have been specifically designed for safe use in the MRI environment and the first experience reported suggests that the technology is safe and may allow patients to undergo MRI.
