Community-acquired pneumonia represents the third-highest cause of mortality in industrialized countries and the first due to infection. Although guidelines for the approach to this infection model are widely implemented in international health schemes, information continually emerges that generates controversy or requires updating its management. This paper reviews the most important issues in the approach to this process, such as an aetiologic update using new molecular platforms or imaging techniques, including the diagnostic stewardship in different clinical settings.

Article Synopsis
  • Liver SBRT is being evaluated for its effectiveness in accurately positioning patients for treatment, focusing on comparing fiducial markers against liver contours and bone references.
  • A study involving 18 patients analyzed the differences in displacement when using these methods, revealing that fiducials were more reliable in positioning.
  • Results indicated that liver contouring yielded smaller displacements compared to bone references and was nearly as precise as fiducials, suggesting it could be a viable alternative when fiducials aren't used.

Article Synopsis
  • The study investigates whether magnetic resonance imaging (MRI) provides better visualization of hepatocellular carcinoma compared to computed tomography (CT) for delineating the gross tumor volume (GTV) in liver stereotactic radiotherapy.
  • Eight radiation oncologists evaluated CT and MRI scans to define GTV volumes in five patients, with results showing minor differences in average volume delineation between the two imaging modalities.
  • The findings suggest that while CT is sufficient for well-defined tumors, MRI can be beneficial for cases where tumors are not clearly visible, highlighting significant interobserver variability in GTV delineation.

Article Synopsis
  • The study investigates the timing of intubation in patients with SARS-CoV-2 pneumonia, suggesting that delaying intubation increases the risk of mortality, especially in cases of acute respiratory failure.
  • Data from 4,198 patients across 73 ICUs revealed that patients who were intubated earlier (within 24 hours) experienced lower mortality rates compared to those who were intubated later (37% vs. 32%).
  • The findings support early intubation as a protective factor against death in COVID-19 pneumonia patients, emphasizing the importance of timely intervention in critical care settings.

Background And Aim: The optimal imaging test for gross tumor volume (GTV) delineation in non-spine bone metastases has not been defined. The use of stereotactic body radiotherapy (SBRT) requires accurate target delineation. Magnetic resonance imaging (MRI) and/or fludesoxyglucose positron emission tomography (18FDG-PET) allow for better visualization of the extent of bone metastases and optimizes the accuracy of tumor delineation for stereotactic radiotherapy compared to computed tomography (CT) alone.

Introduction: The standard therapy for locally advanced rectal cancer (LARC) is based on neoadjuvant chemoradiotherapy (nCRT) with fluoropyrimidines. There are different biomarkers used as prognostic factors in these tumors. Some studies advocate the use of the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR) as prognostic factors in this clinical scenario.

Background: Approximately 30% of patients with localized prostate cancer (PCa) who undergo radical prostatectomy will develop biochemical recurrence. In these patients, the only potentially curative treatment is postoperative radiotherapy (PORT) with or without hormone therapy. However, the optimal radiotherapy dose is unknown due to the limited data available.

Summary: TiFoSi (Tissues Forces & Signaling) is an efficient computational tool for performing mechanobiology simulations of planar epithelia. A drawback of this tool is that it relies on an XML configuration file (input data) that can be cumbersome to set up and/or decode due to the endless possibilities of the software. Moreover, some modeling know-how is needed in order to provide equations that describe gene regulatory interactions.

Background: Procalcitonin (PCT) and C-Reactive Protein (CRP) are useful biomarkers to differentiate bacterial from viral or fungal infections, although the association between them and co-infection or mortality in COVID-19 remains unclear.

Methods: The study represents a retrospective cohort study of patients admitted for COVID-19 pneumonia to 84 ICUs from ten countries between (March 2020-January 2021). Primary outcome was to determine whether PCT or CRP at admission could predict community-acquired bacterial respiratory co-infection (BC) and its added clinical value by determining the best discriminating cut-off values.

Inherited retinal diseases (IRD) are a group of heterogeneous disorders, most of which lead to blindness with limited therapeutic options. Pathogenic variants in RBP4, coding for a major blood carrier of retinol, retinol-binding protein 4, are responsible for a peculiar form of IRD. The aim of this study was to investigate if retinal function of an RBP4-related IRD patient can be improved by retinol administration.

Despite the fact that the last year has been marked by the SARS-CoV-2 pandemic, there have been many articles published on non-COVID pneumonia. Making the selection has not been easy, having based on those articles that we think can bring us some novelty and help in clinical practice. We have divided the selection into seven sections: patient severity, diagnosis, treatment, ventilation, novelties in the guidelines, fungal infection and organ donation.
