Real-world data on the use of lipid-lowering therapy (LLT) in clinical practice show that about 80% of (very) high-cardiovascular (CV)-risk patients disregard the 2019 European Society of Cardiology (ESC) Guidelines' recommendations on dyslipidemias. The availability of proprotein convertase subtilisin/kexin type 9 monoclonal antibodies (PCSK9mAb) should reduce this gap. Our aim was to provide data on PCSK9mAb use in clinical practice, investigating the achievement of the ESC Guidelines' recommendations in the real world.

Article Synopsis
  • The 2023 ESC update to the HF guidelines emphasizes two new drugs, SGLT2 inhibitors and finerenone, for preventing heart failure in patients with diabetic chronic kidney disease, and supports the use of SGLT2 inhibitors across all levels of left ventricular ejection fraction.
  • Quadruple therapy in patients with reduced ejection fraction is beneficial, and the "high-intensity care" approach, involving rapid medication adjustments and close monitoring post-acute heart failure, leads to better outcomes.
  • Recent trials suggest that semaglutide can improve quality of life and physical activity in obese patients with preserved ejection fraction, while findings on additional diuretics and therapies, especially for different

Article Synopsis
  • A study aimed to validate the multi-domain definition of frailty proposed by the HFA-ESC in patients with heart failure, highlighting its high prevalence.
  • The analysis of 854 patients revealed that the risk of death and hospitalization increased with the number of frailty domains fulfilled, indicating a direct relationship between frailty and adverse health outcomes.
  • The Frailty Index (FI) demonstrated slightly better effectiveness than the domain count in predicting mortality risk, showcasing its potential as a useful tool for assessing frailty in heart failure patients.

Article Synopsis
  • Hospitalization for heart failure significantly affects outcomes in patients with advanced heart failure, with inpatients showing worse overall health compared to outpatients.
  • In a study of 1149 patients, those hospitalized at enrollment had a higher one-year all-cause mortality or heart failure hospitalization rate (50.9%) than outpatients (36.8%).
  • The findings suggest that hospitalization indicates a poor prognosis, highlighting the need for targeted interventions like mechanical support or heart transplantation for these high-risk patients.

Article Synopsis
  • The study focuses on patients with advanced heart failure (HF), specifically analyzing the impact of ischemic etiology on prognosis within this high-risk group, based on the updated 2018 HFA-ESC criteria.
  • Ischemic causes were found to be the most common type of heart failure in patients with both advanced and nonadvanced HF, with statistically significant differences in hospitalization and mortality rates.
  • The research indicates that patients with ischemic heart failure among these groups face a greater risk of death and hospitalization when compared to those with nonischemic causes, emphasizing the need for targeted care strategies.

Heart failure (HF) with preserved ejection fraction (HFpEF) causes a progressive limitation of functional capacity, poor quality of life (QoL) and increased mortality, yet unlike HF with reduced ejection fraction (HFrEF) there are no effective device-based therapies. Both HFrEF and HFpEF are associated with dysregulations in myocardial cellular calcium homeostasis and modifications in calcium-handling proteins, leading to abnormal myocardial contractility and pathological remodelling. Cardiac contractility modulation (CCM) therapy, based on a pacemaker-like implanted device, applies extracellular electrical stimulation to myocytes during the absolute refractory period of the action potential, which leads to an increase in cytosolic peak calcium concentrations and thereby the force of isometric contraction promoting positive inotropism.

Inflammation characterizes all stages of atherothrombosis and provides a critical pathophysiological link between plaque formation and its acute rupture, leading to coronary occlusion and heart attack. In the last 20 years the possibility of quantifying the degree of inflammation of atherosclerotic plaques and, therefore, also of vascular inflammation aroused much interest. Fluoro-deoxy-glucose photon-emissions-tomography (F-FDG-PET) is widely used in oncology for staging and searching metastases; in cardiology, the absorption of F-FDG into the arterial wall was observed for the first time incidentally in the aorta of patients undergoing PET imaging for cancer staging.

Leadless pacemaker implantation (LPI) has fewer device complications and reduced chance of infection compared to conventional pacemakers. Dextrocardia with situs viscerum inversus (DC+SVI) is a rare condition, which seldom leads to cardiac complications. However, its presence poses a challenge to operators in cardiac procedures.

Background: The growing size of the end stage renal disease (ESRD) population highlights the need for effective dialysis access. Exhausted native vascular access options have led to increased use of catheters and prosthetic shunts, which are both associated with high risks of access failure and infection. Emerging alternatives include tissue-engineered vascular grafts (TEVG).

Rituximab (RTX), an anti-CD20 monoclonal antibody, has shown to be an effective induction treatment for small-vessel vasculitides associated with antineutrophil cytoplasm antibodies (AAV) in both newly diagnosed and relapsing patients. However, the role of RTX in the management of the most severe cases of AAV remains to be fully elucidated. The aim of this study was to assess both safety and efficacy of an intensified B-cell depletion therapy (IBCDT) protocol, including RTX, cyclophosphamide (CYC), and methylprednisolone pulses without additional maintenance immunosuppressive therapy in a cohort of 15 AAV patients with the most severe features of AVV renal involvement (as <15 ml/min GFR and histological findings of paucimmune necrotizing glomerulonephritis with more than 50% crescents of non-sclerotic glomeruli at the renal biopsy).

Women undergoing stem cell transplantation (SCT) are mostly young and have more than 90% probability of ovarian failure, which is often permanent. A woman's age, use of radiotherapy and alkylating chemotherapy, and the allogeneic type of transplant are associated with a higher rate of premature ovarian failure and worse residual ovarian function. Premature ovarian failure has serious systemic and psychological effects that may need treatment and should be managed by practitioners trained to treat this particular population of women.
