Article Synopsis
  • - The study investigates cutaneous (CL) and mucosal leishmaniasis (ML) cases in mainland Portugal from 2010 to 2020, identifying 43 CL and 7 ML cases, predominantly in immunosuppressed individuals.
  • - Most cases were autochthonous, with common symptoms including papules/nodules, particularly on the head, and a varied treatment approach noted, mainly using liposomal amphotericin B for ML.
  • - The findings highlight a need for better surveillance, clinical awareness, and diagnostic capabilities for leishmaniasis, advocating a One Health approach to tackle the complexities of the disease in Portugal and globally.

Article Synopsis
  • Tuberculosis (TB) remains a major health issue globally, highlighted by a complex case of a 24-year-old Angolan patient with symptoms like fever and neurological issues, complicated by chronic corticosteroid use which can reactivate latent TB.
  • The case involves diagnostic challenges as the patient had a previous Kikuchi's disease diagnosis and showed paradoxical worsening despite receiving appropriate TB treatment, with miliary TB potentially affecting various organs and presenting difficulties in identification.
  • Emphasizing the need for high suspicion of TB in immunocompromised patients, the report discusses the importance of thorough microbiological testing, long treatment durations, and the role of corticosteroids in managing the disease effectively.

Introduction During the coronavirus disease 2019 (COVID-19) pandemic, a high number of patients needed to be admitted to the intensive care units (ICUs). Such a high demand led to periods where resources were insufficient and the triage of patients was needed. This study aims to evaluate the performance of the Acute Physiology and Chronic Health Evaluation (APACHE) II as a predictor of mortality in periods where triage protocols were implemented.

Diffuse alveolar haemorrhage related to an anti-neutrophil cytoplasmic antibody (ANCA)-associated small vessel vasculitis is an extremely rare form of presentation of breast cancer. Here we report the case of a 77-year-old woman with a histological diagnosis of a papillary ductal carcinoma of the breast presenting with a diffuse alveolar haemorrhage secondary to a perinuclear ANCA-associated vasculitis. To our knowledge, this is the first case ever reported of a diffuse alveolar haemorrhage related to an ANCA-associated small vessel vasculitis as a form of presentation of breast cancer.

Background: Although there is some evidence suggesting that certain viruses may be involved in the onset of inflammatory bowel disease (IBD), data regarding viral prevalence and viral load in blood and mucosa of patients with IBD are scarce. The main aim of this study is to evaluate the prevalence and viral load of common Epstein-Barr virus (EBV), cytomegalovirus (CMV), and human herpes virus 6 in blood and mucosa of adult patients with endoscopic active IBD.

Methods: From January to December 2014, ulcerative colitis and Crohn's disease patients with active endoscopic disease were consecutively enrolled.
