Article Synopsis
  • Patients with diabetes face higher mortality rates from COVID-19, and while dexamethasone is commonly used to treat severe cases, it may negatively impact blood sugar levels, necessitating a closer look into its effects in this population.
  • This study analyzed data from over 3,600 hospitalized diabetic patients with COVID-19 to compare outcomes between those treated with dexamethasone and those who weren't.
  • The findings revealed no significant differences in survival rates or time to recovery between the two groups, indicating that dexamethasone may not be beneficial for diabetic patients with COVID-19 and highlighting the need for further research.

Article Synopsis
  • - The hypothalamus-pituitary axis (HPA) is an uncommon site for metastasis from non-Hodgkin's lymphoma, accounting for less than 0.5% of such cases.
  • - The patient exhibited panhypopituitarism and severe hyponatremia linked to the syndrome of inappropriate antidiuretic hormone (SIADH), along with sudden diabetes insipidus (DI), indicating possible hypothalamic infiltration.
  • - While chemotherapy that can penetrate the blood-brain barrier is the main treatment, surgery and radiotherapy have not shown significant improvements in survival rates, and the overall prognosis remains poor.

Objective: Our aim in this study was to determine whether there are differences in glycemia during wound and wound-free states among individuals with diabetes at a multidisciplinary diabetic foot and wound clinic from 2012 to 2019.

Methods: We conducted a retrospective analysis of prospectively collected data over 7.4 years from the Johns Hopkins Multidisciplinary Diabetic Foot and Wound Clinic.

"First-generation" somatostatin receptor agonists (SSTRAs) octreotide and lanreotide are the most commonly used first-line pharmacological therapy for patients with acromegaly. A subset of patients respond only partially or not at all to the first-generation SSTRA, necessitating the use of additional pharmacological agents or other modes of therapy. Pasireotide is a "second-generation" SSTRA that has multi-receptor activity.

We describe a case of a Black female patient with a history of type 2 diabetes mellitus and systemic lupus erythematosus, who had a subacute onset of severe hypoglycemia that persisted after cessation of insulin therapy. Biochemical testing revealed hyperinsulinemic hypoglycemia, normal serum triglycerides, and high-normal serum adiponectin levels. Abdominal imaging demonstrated an 11-mm cystic pancreatic lesion.

Article Synopsis
  • * A case study describes a previously healthy woman who experienced severe heart failure due to a spontaneous bleed in a pheochromocytoma, resulting in a pheochromocytoma crisis and stress cardiomyopathy, but she recovered quickly with acute medical treatment.
  • * The text emphasizes the importance of recognizing pheochromocytomas as a potential cause of acute heart failure, and highlights the need for careful evaluation and appropriate management strategies to avoid complications during critical care.

Article Synopsis
  • - Lipodystrophy syndromes are rare disorders marked by the loss of fat tissue, leading to issues like insulin resistance and high blood lipid levels, and can often be misdiagnosed due to varying symptoms.
  • - Correct identification of these syndromes is crucial for effective treatment and ongoing care, as many cases are overlooked during physical exams by healthcare providers.
  • - The potential use of GLP-1 analogs for treatment hasn't been thoroughly studied, and this text aims to raise awareness among general healthcare practitioners about recognizing partial lipodystrophy and exploring treatment options.

The obesity epidemic in aging populations poses significant public health concerns for greater morbidity and mortality risk. Age-related increased adiposity is multifactorial and often associated with reduced lean body mass. The criteria used to define obesity by body mass index in younger adults may not appropriately reflect age-related body composition changes.

Purpose Of Review: Omega-3 fatty acids (n-3 FA) lower triglycerides, have anti-inflammatory properties, and improve metabolism. Clinical evidence of cardiovascular benefit with omega-3 fatty acids is mixed. We discuss mechanisms providing biological plausibility of benefit of omega-3 fatty acids in cardiovascular risk reduction and review clinical trials investigating the benefits of prescription omega-3 fatty acids in dyslipidemia, atherosclerotic cardiovascular disease (ASCVD), and diabetes.

Male hypogonadism affects 10-30% of the male population and is often under-recognized and under-treated. Different replacement formulations exist, each with specific benefits and limitations. These replacements include gels, patches and short- and long-acting injectables.

Purpose Of The Review: Time-restricted eating (TRE) is a promising dietary intervention for weight loss and improvement of cardiometabolic risk factors. We aim to provide a critical review of blood pressure outcomes reported in clinical TRE studies in adults with metabolic syndrome, in the context of the proposed mechanisms that underlie the relationship between timing of eating and blood pressure.

Recent Findings: Clinical TRE studies report mixed results pertaining to blood pressure outcomes, likely due to significant heterogeneity in study design and TRE protocols.

Purpose Of Study: Diabetes mellitus accelerates the development of atherosclerosis. Patients with diabetes mellitus have higher incidence and mortality rates from cardiovascular disease and undergo a disproportionately higher number of coronary interventions compared to the general population. Proper selection of treatment modalities is thus paramount.
