Article Synopsis
  • - In the early 1990s, Medicare saw a rapid increase in postacute care (PAC) providers, but spending grew even faster than the number of providers.
  • - By the late 1990s, Congress mandated the creation of prospective payment systems (PPSs) for PAC to control spending; however, the new systems unintentionally led to greater spending growth and high profits for providers.
  • - The article highlights MedPAC's evaluation of payment adequacy in PAC for 2006 and 2007, outlines issues with the PPSs that cause payment and cost mismatches, and reviews studies comparing patient assessments and outcomes for joint-replacement patients across different PAC settings.
