Primary sinonasal tuberculosis remains a diagnostic challenge as it mimics sinonasal granulomatous disease or neoplasms. Its characteristic signs and symptoms may be non-specific and highly variable. Here we report a unique clinical condition of a 24-year-old man who presented with unilateral nasal obstruction and epiphora for 2 years.

Congenital cervical midline cleft (CCMC) is a rare disease of the head and neck region with only a few reported series. The patients attending the ENT or Plastic surgery OPD of a tertiary referral hospital of eastern India were screened for the presence of CCMC. All the patients found to have the developmental abnormality were included in the study and treated.

It was found that the arrow was penetrating through the nasal bones. An xray skull lateral view showed the tip of the arrow penetrating into the posterior wall of the sphenoid sinus.
