Investigating barriers to accident precursor reporting in East Azerbaijan Province Gas Company from the perspective of HSE officers: a qualitative study.

Investigating the root causes of under-reporting near misses is very important. The aim of this study was investigating barriers to near-miss reporting in East Azerbaijan Province Gas Company from the perspective of health, safety and environment (HSE) officers. Semi-structured individual interviews were used and 21 interviews were conducted with HSE officers.

Prioritizing the indicators influencing permit to work system efficiency based on an analytic network process.

A permit to work (PTW) system is a formal procedure designed to control non-routine and hazardous works. However, this system by itself does not prevent incidents, and various factors contribute to its efficiency. This study aims to prioritize the indicators influencing system efficiency.

Investigating the status of accident precursor management in East Azarbaijan Province Gas Company.

The accident precursor reporting system is a key point in preventing accidents and proper functioning of this system is critical. In the present study, the accident precursor reporting system in the Iranian East Azarbaijan Province Gas Company was investigated. This quantitative-qualitative study used field survey and document review methods to determine how the company was reporting.
