Background: Recurrent burn contractures on the volar aspect of the hand present a formidable challenge, especially in the pediatric patient. We used Integra followed by staged full-thickness skin grafting for recurrent bilateral volar hand burn contracture in a toddler. We reviewed the literature to appraise the utility of full-thickness skin graft (FTSG) combined with Integra used for volar hand contractures.

Reducing the Risk: An Evidence-Based Approach to the Febrile Infant Less Than 60 Days of Age in the Emergency Department.

Fever is the most common complaint for children of all ages who present to an emergency department (ED) for evaluation and treatment. Although most children with fever have a self-limiting viral illness, the infant less than 60 days of age with fever poses a higher risk for an underlying serious bacterial illness (SBI) including urinary tract infections, bacteremia and bacterial meningitis, or an invasive bacterial infection (IBI) including bacteremia. This case is a discussion of a high-risk febrile infant that was evaluated in the emergency department and was ultimately diagnosed with Streptococcus B meningitis.

Objective: This study aims to evaluate the success rates and complications associated with different free flap donor sites used in surgical reconstruction for mandibular osteoradionecrosis, providing insights to aid clinical decision-making.

Objective: To analyse amniotic fluid volume (AFV), specifically oligohydramnios or polyhydramnios, and associated pregnancy and neonatal outcomes in twin gestations through systematic review and meta-analysis.

Methods: We utilised systematic review methodology to identify items within published and grey literature resources. Prospective and retrospective studies with a control group were included.

Objective: To analyze outcomes of singleton pregnancies with idiopathic polyhydramnios through a systematic review and meta-analysis.

Methods: Electronic databases, including MEDLINE, OVID, EBSCO, Cochrane collection and Science Citation Index, were searched from 1946 to 2019. Gray literature and tables of contents of relevant journals were also screened.

Introduction: Asthma is the most prevalent chronic disease of childhood. Caregiver knowledge is essential to optimize patient quality and safety. Asthma education is an opportunity that can lead to decreased morbidity and mortality, reduced admission rates, lower cost, and improved outcomes for a child with chronic asthma.

Significance of single β-mannanase supplementation on performance and energy utilization in broiler chickens, laying hens, turkeys, sows, and nursery-finish pigs: a meta-analysis and systematic review.

This review will give a brief description of β-mannans, abundance in feedstuffs, utility of supplemental feed β-mannanase, and subsequent animal responses. Soybean products and co-products of processing palm, coconut, and guar seeds are the major sources of β-mannans in poultry and livestock feed. β-Mannans are linear polymers of mannose residues linked by β-1,4 glycosidic bonds and their ingestion elicit undesirable and metabolically costly responses.

Implementing Safe and Effective Pediatric Procedural Sedation in the Emergency Department.

There has been a great increase in the number of diagnostic and therapeutic procedures performed in the emergency department (ED) setting over the past several decades. Most of these procedures are unscheduled, unplanned, and much more likely to be successful and result in a positive outcome when the child's pain, fear, anxiety, and movement are controlled. To promote patient quality and safety, increase the effectiveness and efficiency of care, and improve patient and caregiver satisfaction, there has been a proportionate increase in the number of pediatric procedural sedations performed in the ED.

Telehealth for genetic counseling: A systematic evidence review.

Telehealth options, such as telephone counseling or videoconferencing, for service delivery in genetic counseling are becoming more widely accepted. However, until now, there has not been a systematic review of the literature focused specifically on genetic counseling outcomes for telehealth. We performed a systematic evidence review to compare telehealth genetic counseling (THGC), including videoconferencing and telephone counseling, across specialties to in-person genetic counseling (IPGC) for a range of outcomes specific to patient and provider experiences and access to care.

Best Practices for Community-Engaged Research with Pacific Islander Communities in the US and USAPI: A Scoping Review.

This scoping review identifies the best practices of community-based participatory research with Pacific Islanders in the United States and United States Affiliated Pacific Islands. Eighty-four articles from January 2000 to December 2017 were included in the review. Best practices included the importance of engaging Pacific Islander community leaders as research staff, community co-investigators, and community advisory board members.

Outcomes of disease prevention and management interventions in food pantries and food banks: a scoping review.

Objective: Food insecurity affects millions of Americans and is associated with a range of adverse health outcomes. Food insecure individuals often obtain food from food pantries/banks, prompting health researchers to implement disease prevention/management interventions at these sites. This review examined the existing peer-reviewed research on disease prevention/management interventions implemented in food pantries/banks.

Pediatric Sexual Abuse: An Interprofessional Approach to Optimizing Emergency Care.

Introduction: Pediatric sexual abuse can lead to devastating lifelong consequences for the child, their caregivers, and society at large. To achieve the goal of optimizing patient outcomes, strategies to enhance teamwork, education, and mentorship are essential. The ultimate goal of child safety and protection through an evidence-based approach will only be attained when interprofessional alignment of expertise and resources is implemented.

Mutational processes shape the landscape of TP53 mutations in human cancer.

Unlike most tumor suppressor genes, the most common genetic alterations in tumor protein p53 (TP53) are missense mutations. Mutant p53 protein is often abundantly expressed in cancers and specific allelic variants exhibit dominant-negative or gain-of-function activities in experimental models. To gain a systematic view of p53 function, we interrogated loss-of-function screens conducted in hundreds of human cancer cell lines and performed TP53 saturation mutagenesis screens in an isogenic pair of TP53 wild-type and null cell lines.

Recent studies have highlighted the promise of targeting tumor neoantigens to generate potent antitumor immune responses and provide strong motivation for improving our understanding of antigen-T-cell receptor (TCR) interactions. Advances in single-cell sequencing technologies have opened the door for detailed investigation of the TCR repertoire, providing paired information from TCRα and TCRβ, which together determine specificity. However, a need remains for efficient methods to assess the specificity of discovered TCRs.

Introduction: Food insecurity is a difficulty faced in many households. During periods of food insecurity, households often seek food supplied by food pantries and food banks. Food insecurity has been associated with increased risk for several health conditions.

Challenges and Strategies of Sexual Minority Youth Research in Southwest Virginia.

A research team that works on scholarship related to sexual minority issues has been conducting research and recruiting participants since the fall of 2012. In the years since the team was formed, there has been a variety of challenges with recruitment, approval for research through the Institutional Review Board, gaining access to the lesbian, gay, bisexual, and queer (LGBQ) community, and general mistrust of researchers. Using concepts from feminist-informed qualitative research methodology-including reflexivity, positionality, and engaging in research with individuals from marginalized communities-we present reflections on some of the methodological challenges the research team has encountered while trying to conduct LGBQ research in southwest Virginia.

Most and Least Meaningful Learning Experiences in Marriage and Family Therapy Education.

Marriage and family therapy educators increasingly emphasize training competencies. What we know less about is what makes family therapy education meaningful to marriage and family therapy (MFT) graduate students and what does not. In this study, through an Internet survey, we explored the most and least meaningful learning experiences of 68 MFT graduate students and recent graduates of Commission on Accreditation for Marriage and Family Therapy Education-accredited programs.

Comprehensive analysis of cancer-associated somatic mutations in class I HLA genes.

Detection of somatic mutations in human leukocyte antigen (HLA) genes using whole-exome sequencing (WES) is hampered by the high polymorphism of the HLA loci, which prevents alignment of sequencing reads to the human reference genome. We describe a computational pipeline that enables accurate inference of germline alleles of class I HLA-A, B and C genes and subsequent detection of mutations in these genes using the inferred alleles as a reference. Analysis of WES data from 7,930 pairs of tumor and healthy tissue from the same patient revealed 298 nonsilent HLA mutations in tumors from 266 patients.
