Introduction: In the absence of appropriateness specific guidelines, one important cause of health resources waste could be overuse of diagnostic procedures. Since arterial hypertension is a very frequent disease there could be such a risk in its management.

Aim: To evaluate the prescriptive appropriateness of non-invasive diagnostic tests (echocardiography, carotid ultrasound, ECG exercise test, 24 h Ambulatory Blood Pressure Monitoring-ABPM) in a primary and secondary prevention outpatient's service.

Article Synopsis
  • The study investigates the relationship between vitamin D levels and lipid profiles in children and adolescents with obesity, highlighting the high prevalence of vitamin D deficiency in this group compared to those with normal weight.
  • Results show that individuals with low vitamin D levels had significantly worse indicators of metabolic health, including higher glucose levels, insulin resistance, and unfavorable lipid ratios.
  • The findings suggest that vitamin D deficiency is a modifiable risk factor for cardiometabolic issues, indicating the need for early intervention and preventive strategies.

  • Preclinical research over the past 35 years has shown that glial cells, like astrocytes and microglia, play a critical role in brain health and diseases, impacting neurodegeneration and pain.
  • Several potential drug targets have been identified, including receptors and enzymes involved in neuroinflammation, but translating these findings into clinical treatments faces significant challenges.
  • The review will focus on the role of glial cells in human diseases, discussing findings from post-mortem studies and human cell models, as well as the potential for visualizing glial responses to neuroinflammation in patients.

Article Synopsis
  • Osteoarthritis (OA) is a common musculoskeletal disease that causes chronic pain, significantly impacting patients' quality of life, especially in older populations.
  • Chronic pain from OA is linked to emotional issues like anxiety and depression, which may be tied to neuroinflammation in the nervous system.
  • Research using the MIA model in rodents shows that neuroinflammation could be a key factor in both OA-related pain and mood disorders, suggesting potential new treatment targets involving specific cells and molecules.

Introduction: Vitamin D is an essential hormone for humans, playing an important role in musculoskeletal and calcium homeostasis. Its deficiency/insufficiency seems to contribute to the development of cardiometabolic diseases in adults: this correlation appears less clear for children and adolescents. The aim of this paper was to review literature data on the relationship between vitamin D and lipid profile alterations in pediatric population.

A rapid, cost-effective, and simple nucleic acid isolation technique coupled with a point-of-need DNA amplification assay is a desirable goal for programmatic use. For diagnosis of Visceral Leishmaniasis (VL), Recombinase Polymerase Amplification (RPA) rapid tests for the detection of Leishmania DNA are versatile and have operational advantages over qPCR. To facilitate the delivery of the RPA test at point-of-need for VL diagnosis, we compared two rapid DNA extraction methods, SwiftDx (SX) and an in-house Boil and Spin (BS) method, coupled with RPA amplification, versus more widely used methods for DNA extraction and amplification, namely Qiagen (Q) kits and qPCR, respectively.

Acute renal failure (AKI) is a high-prevalence complication in patients with cancer. The risk of AKI after cancer diagnosis is 18% in the first year, 27% in the fifth year, and 40% of critically ill patients with cancer require renal replacement therapy. The causes of AKI may be pre-renal due to hemodynamic problems, related to the cancer, metabolic complications, and drug or surgical treatment.

Hereditary predisposition to cancer affects about 3-5% of renal cancers. Testing criteria have been proposed in France for genetic testing of non-syndromic renal cancer. Our study explores the detection rates associated with our testing criteria.

Background: The advent of elexacaftor/tezacaftor/ivacaftor (ETI) resulted in unprecedented clinical benefits for eligible adults with CF. As a result, the question of whether chronic treatments can be safely stopped or adapted to this new situation has become a matter of great interest. Our objective was to derive a consensus among Italian experts on the impact of ETI on the current clinical management of CF lung disease.

Article Synopsis
  • * The main findings indicated that 14.8% of patients suffered from unfavorable outcomes due to significant complications or low quality-of-life scores post-surgery, with most patients showing good satisfaction overall.
  • * Logistic regression revealed that middle-aged men faced a higher risk of dissatisfaction after PPI compared to younger and older patients, while other factors like erectile dysfunction cause and surgical volume didn't significantly affect outcomes.

Background: Peyronie's disease (PD) represents a challenging urological disease, due to not optimal post-operative surgical outcomes. We aim to retrospectively evaluate if vacuum erection device (VED) treatment before penile curvature surgery is able to improve post-operative surgical outcomes.

Methods: All enrolled patients were assigned to the following groups: (a) the treatment group: VED treatment (three times per week) starting 3 months before surgery and (three times per week) one month after surgery; and (b) the control group: VED treatment (three times per week) one month after surgery.

Feeding is one of the most important factors influencing production efficiency and the environmental impact of livestock production. This study evaluates the possibility of reducing the impact of beef cattle production by optimizing the fertilization management of home-grown forage on the same farms. To this end, two scenarios were compared on two beef cattle farms in northern Italy, a baseline scenario (BS) and a scenario with optimized management (OMS) in terms of nitrogen fertilizer use.

: The peak of incidence of testicular cancer (TC) occurs among individuals in their reproductive age, emphasizing the importance of fertility preservation as an integral aspect of disease management. Sperm cryopreservation performed before orchiectomy is ineffective in azoospermic men, necessitating alternative approaches such as microdissection testicular sperm extraction (mTESE) at the time of orchiectomy (onco-mTESE) to obtain viable sperm. This study presents the findings from our institution's experience with onco-mTESE and critically discusses our results in light of the existing body of literature.

Article Synopsis
  • Research on the connection between gut microbiota and the central nervous system (CNS) has significantly increased, revealing the impact of gut-brain communication on various health issues, especially chronic pain.
  • Studies indicate that changes in gut microbiota can influence glial cells in the CNS, which play a key role in pain and neurodegenerative diseases.
  • The review discusses how alterations in microbiota can affect glial cell function and morphology, suggesting that these interactions might contribute to the onset and persistence of chronic pain.

Background: The potential reservoirs of visceral leishmaniasis (VL) in South Asia include asymptomatic and relapsed cases of VL, along with patients with post kala-azar dermal leishmaniasis (PKDL). Accordingly, accurate estimation of their parasite load is pivotal for ensuring disease elimination, presently targeted for 2023. Serological tests cannot accurately detect relapses and/or monitor treatment effectiveness, and therefore, parasite antigen/nucleic acid based detection assays remain the only viable option.

Purpose: To study the post-operative refractive error (RE) of patients undergoing combined phaco-vitrectomy and to find out which intraocular lens (IOL)-power formula had the best refractive outcomes.

Methods: In this retrospective multicentric study we compared the preoperative expected target with the postoperative RE of patients undergoing combined phaco-vitrectomy due to vitreomacular traction, macular pucker, full thickness macular hole or lamellar macular hole. A multinomial logistic regression was performed to compare the postoperative REs and the differences between expected and postoperative REs among the SRK-T, Olsen's and Holladay-2 formulas.
