The article describes a non-typical access to treatment of gigantic incarcerated inguinal hernias. Preperitoneal suprainguinal access allows solution of the main task of operative aid in this pathology: inspection of the strangulated organs, safe dissection of the incarcerating ring and prosthesis of the anterior abdominal wall. This method in many aspects is comparable with traditional methods of hernioplasty, but the postoperative quality of life is much better.

35 patients with arterial hypertension and obesity against the background of fasting-diet therapy and after 1 and 6 months after treatment conducted daily monitoring of blood pressure, microalbuminuria and glomerular filtration rate, the study of color and contrast sensitivity of retinal eyes. Fasting-diet therapy within 11 days results in reliable reduced daily average AD and stabilization of load pressure indicators; reduction originally pathological microalbuminurii at 18%, increase in the number of patients with normal speed glomerular filtering 48%; improving of eyes function, these changes are saved within 1-6 months after treatment without the using of antihypertensive therapy.

Metabolic therapy is one of several therapeutic strategies that can be used in patients with lower limb chronic obliterative arterial disease (COAD). Tanakan (EGb76) has demonstrated high efficiency and safety in 9 controlled clinical trials and author's clinical series in patients with lower limb COAD. Improvement with significant increase of pain-free walking distance and beneficial safety profile was observed in all cases.
