Ten-year follow-up of degenerative spinal lesions on radiographs and MRI in axial spondyloarthritis: results of the DESIR (DEvenir des spondylarthropathies indifférenciées récentes) cohort.

Objectives: To investigate the occurrence of spinal degenerative lesions (DL)s in axial spondyloarthritis (axSpA) inception cohort in radiographs and MRI over 10 years (10Y), to assess their changes over time and factors associated with them.

Methods: Whole spine MRI and cervical and lumbar spine radiographs at baseline/5Y/10Y of patients with axSpA from the DESIR cohort were assessed for DLs by three readers. For descriptive analyses, DLs were defined by agreement between ≥ 2/3 readers or using the average of their assessments, at the patient level (≥ 1 lesion/patient).

Efficacy of upadacitinib in subgroups of patients with axial spondyloarthritis with early versus established disease.

Objectives: Early disease activity control with targeted therapies may improve long-term outcomes in axial spondyloarthritis (axSpA). Here, we evaluated the efficacy of upadacitinib in patients with axSpA with shorter versus longer symptom durations.

Methods: SELECT-AXIS 1 and 2 studies enrolled patients with radiographic axSpA (r-axSpA) and non-radiographic axSpA (nr-axSpA) naïve to biologic disease-modifying antirheumatic drugs (bDMARD-naïve) and with an intolerance or inadequate response to bDMARD therapy.

The Assessment of SpondyloArthritis International Society (ASAS) Consensus-Based Expert Definition of Difficult-to-Manage, including Treatment-Refractory, Axial Spondyloarthritis.

Objectives: To develop a consensus-based expert definition of difficult-to-manage (D2M) axial spondyloarthritis (axSpA), incorporating treatment-refractory (TR) disease.

Methods: A literature review was conducted in 2022 to identify potential definitions for D2M/TR axSpA from prior studies, followed by a 2-round Delphi consensus process conducted in 2022 and 2023 to identify components of D2M axSpA. Based on the results of the Delphi process, a draft of the D2M axSpA definition was developed and presented to the expert task force, including patient representation, and, subsequently, to the Assessment of SpondyloArthritis International Society (ASAS) membership for endorsement in January 2024.
