Causes of Unwarranted Variation and Disparity in Breast Cancer Management in Regional and Rural Area.

Introduction: Breast cancer management is complex, requiring personalised care from multidisciplinary teams. Research shows that there is unwarranted clinical variation in mastectomy rates between rural and metropolitan patients; that is, variation in treatment which cannot be explained by disease progression or medical necessity. This study aims to determine the clinical and nonclinical factors contributing to any unwarranted variation in breast cancer management in rural patients and to evaluate how these factors and variations relate to patient outcomes.

Force-Activated Spin-Crossover in Fe and Co Transition Metal Mechanophores.

Transition metal mechanophores exhibiting force-activated spin-crossover are attractive design targets, yet large-scale discovery of them has not been pursued due in large part to the time-consuming nature of trial-and-error experiments. Instead, we leverage density functional theory (DFT) and external force explicitly included (EFEI) modeling to study a set of 395 feasible Fe and Co mechanophore candidates with tridentate ligands that we curate from the Cambridge Structural Database. Among nitrogen-coordinating low-spin complexes, we observe the prevalence of spin crossover at moderate force, and we identify 155 Fe and Co spin-crossover mechanophores and derive their threshold force for low-spin to high-spin transition ().

'The upside-down' healthcare professional students' experiences of delirium: an all-Ireland focus group study.

Background: Delirium is a complex neuropsychiatric syndrome characterised by an acute state of confusion, with a substantial impact on medical inpatients. Despite its growing recognition as a global healthcare concern, delirium remains underdiagnosed, partly due to a lack of awareness among healthcare professionals. The aim of this study was to explore how healthcare professional students experience caring for individuals experiencing delirium, the influence of their current pre-registration healthcare education, and importance of interprofessional teamwork in their role.

Minocycline-induced lupus.

Antibiotics are indicated for inappropriately prolonged durations not only for infectious diseases but also for non-infectious dermatologic conditions such as acne vulgaris, but inappropriate use is not uncommon. Inappropriate antibiotic prescription can contribute to the emergence of multidrug-resistant organisms, patient harm such as adverse drug events and increased healthcare costs. We experienced a case involving a late adolescent with acne vulgaris who had been treated with minocycline for more than 2 years, leading to the development of drug-induced lupus, attributed to minocycline.
