Infant misidentification and abduction are recognized as "never" events for hospitals in the United States. As near misses are often unreported, root cause analysis of observed near misses may fail to uncover important contributors. We utilized failure mode and effects analysis to proactively identify and eliminate or reduce the risk of infant misidentification or abduction.

  • Conducted in a university hospital setting, the research involved 110 third-year medical students and looked at the relationship between various student and clerkship characteristics and feedback quality.
  • Results showed that higher student clerkship scores correlated with better perceived feedback quality, but the actual quantity and quality of feedback from the clerkship director were mainly influenced by the number of preceptors providing feedback.
