[Acquired long QT syndrome and cardiac arrest after general anesthesia. Case report and review of literature].

A 30-year-old woman, with no medical history, is operated on for breast implants. In recovery room, an episode of torsade de pointes occurs, progressing to ventricular fibrillation. The ECG after cardiopulmonary resuscitation and conversion to a normal sinus rhythm shows a corrected QT interval prolongation, whereas it is normalized after 48hours.

Totally endoscopic coronary artery bypass grafting on the arrested heart is a prerequisite for successful totally endoscopic beating heart coronary revascularisation.

Telemanipulators have been introduced into cardiac surgery recently expanding the scope of minimally invasive techniques and enabling endoscopic cardiac surgery. Our aim was to evaluate clinical results of totally endoscopic single vessel bypass grafting on the arrested as well as on the beating heart. Since 1999, 44 totally endoscopic single vessel arterial bypass grafting procedures were performed at our institution.

Does poor oxygenation during one-lung ventilation impair aerobic myocardial metabolism in patients with symptomatic coronary artery disease?

One-lung ventilation is used during a variety of surgical procedures, even in patients with pre-existing coronary artery disease. The study purpose was to elucidate if myocardial metabolism crosses the anaerobic threshold under hypoxemia during one-lung ventilation. Therefore, we determined myocardial metabolism as a marker for anaerobic myocardial metabolism in patients with significant multi-vessel coronary artery disease undergoing one-lung ventilation during minimally-invasive coronary artery bypass grafting.

Perfusion strategies for totally endoscopic cardiac surgery.

For most of totally endoscopic cardiac procedures femoro-femoral perfusion techniques are necessary. Use of selective bicaval as well as single venous drainage is described. Furthermore, the use of different intraaortic balloons for aortic occlusion is explained and illustrated.

[Robotic coronary artery surgery: past, present and future].

Minimally invasive endoscopic procedures in cardiac surgery have only become possible since the introduction of telemanipulator systems. In this study we review robotic assisted telemanipulation systems and procedures on beating and arrested heart for total endoscopic revascularization. Robotic surgery is still under development.
