Assessment of the reproducibility of the lactulose H2 breath test as a measure of mouth to caecum transit time.

The lactulose H2 breath test is in use as a simple non-invasive measurement of mouth to caecum transit time, but its reproducibility has never been assessed. We have examined the reproducibility of mouth to caecum transit time in 21 normal subjects using lactulose 10, 15, and 20 g; seven subjects being studied with 10 g and 12 each with 15 and 20 g doses. Transit time decreased with increasing doses of lactulose although the differences were not significant between or within (n = 5) individuals.

Cardiac amyloidosis--diagnosis by 2-dimensional echocardiography.

In the past, the precise diagnosis of amyloid cardiomyopathy without resort to invasive techniques has been difficult. Recent experience with 2-dimensional echocardiography in cardiac amyloidosis has suggested that there are characteristic, and perhaps unique, features that are regularly imaged. We describe a case where usage of this non-invasive technique allowed accurate definition of cardiac involvement by amyloid.

Potentially explosive colonic concentrations of hydrogen after bowel preparation with mannitol.

Hydrogen (H2) concentrations were measured in undiluted samples of intracolonic gas and in expired air in twenty patients undergoing colonoscopy after preparation with either mannitol or castor oil. Potentially explosive intracolonic H2 concentrations were present in six out of ten patients given mannitol, but in none given castor oil. This has important implications for colonoscopic, as well as for surgical, electrosurgery.

Controlled trial of cimetidine in upper gastrointestinal haemorrhage.

One hundred and one patients were studied in a double-blind controlled trial to assess the role of oral cimetidine in preventing the continuation or recurrence of acute upper gastrointestinal haemorrhage from various sources, chiefly peptic ulcer. The dose of cimetidine was 800 mg on entering the study followed by 400 mg six hourly. The source of bleeding was identified endoscopically in 96% of patients, peptic ulcer comprising 70%.
