A 47-year-old woman, who 2.5 years previously had undergone resection of a malignant astrocytoma of the left temporal lobe followed by radiotherapy, was found to have a mass in the left frontal lobe. This showed high uptake of thallium-201 (201Tl) on single-photon emission computed tomography and 11C-methionine on positron-emission tomography, suggesting recurrent tumour.

The authors report the results of microsurgical management of 209 cases of ruptured intracranial aneurysms. The outcome of the surgery is discussed from the point of view of operative timing, preoperative grade, age and the effect of vasospasm. The operative results at the time of discharge were as follows: 137 excellent cases, 18 good, 31 fair, 8 poor and 15 deaths.

This case of angioma in the paraspinal muscles with extension into the epidural tissue was complicated by epidural hematoma in the thoracolumbar region. The hemorrhage was caused by rupture of the part of the angioma lying in the epidural tissue. A spinal bruit was of great value in diagnosing the angioma.

A case of intracranial hypoglossal neurinoma is reported. The diagnostic importance of an enlarged hypoglossal canal with isolated hypoglossal nerve palsy is emphasized. The computed tomography and angiographic findings are described.
