Loxapine in patient with clozapine-resistant psychosis.

Clozapine is recognized as the drug of choice for treatment-refractory schizophrenia, but use may be limited because of strict monitoring requirements and adverse effects including severe neutropenia, seizures, and myocarditis. Loxapine is a first-generation antipsychotic with similarities to clozapine in both structure and receptor binding. This case describes a 57-year-old male with a history of severe paranoid schizophrenia despite treatment with clozapine and other psychotropic agents, who experienced clinical improvement after a cross titration from clozapine to loxapine.

Successful use of carbamazepine in a patient with drug rash with eosinophilia and systemic symptoms.

Drug rash with eosinophilia and systemic symptoms (DRESS) is a serious adverse drug reaction with a high mortality rate. Discontinuation of the causative agent is the primary treatment. History of DRESS may put patients at higher risk of future episodes; however, cross-reactivity between various medications is not well established.

Chemical Kinetic Influences of Alkyl Chain Structure on the High Pressure and Temperature Oxidation of a Representative Unsaturated Biodiesel: Methyl Nonenoate.

The high pressure and temperature oxidation of methyl trans-2-nonenoate, methyl trans-3-nonenoate, 1-octene, and trans-2-octene are investigated experimentally to probe the influence of the double bond position on the chemical kinetics of long esters and alkenes. Single pulse shock tube experiments are performed in the ranges p = 3.8-6.

Fractured neck of femur and contralateral intracerebral lesions.

Ten cases of fractured neck of femur studied at necropsy were found to have established contralateral intracerebral lesions. These were mainly infarcts, and it suggested that the presence of hemiparesis predisposes to a fall on to the affected side and that the body weight acts through the hip joint in such a way as to fracture the femoral neck. This may happen irrespective of the magnitude of the patient's trabecular bone volume, but in osteoporotic subjects the fracture will probably be intertrochanteric, whereas in non-osteoporotic patients it will probably be subcapital.
