Article Synopsis
  • * The patient exhibited symptoms such as hypertension, weight gain, and central obesity; various tests indicated high ACTH levels and hinted at ectopic production from a lung tumor.
  • * After surgery to remove the lung tumor, histological analysis confirmed it was a carcinoid tumor, and the patient's ACTH levels decreased significantly post-operation, highlighting the complexity of diagnosing and managing ACTH-secreting tumors.

Powered lower-limb exoskeletons represent a promising technology for helping the upright stance and gait of people with lower-body paralysis or severe paresis from spinal cord injury. The powered lower-limb exoskeleton assistance can reduce the development of lower-limb muscular fatigue as a risk factor for spasticity. Therefore, measuring powered lower-limb exoskeleton training-induced fatigue is relevant to guiding and improving such technology's development.

Article Synopsis
  • This study investigates the relationship between glycemic control, measured by HbA1c levels, and adverse outcomes in women with pre-gestational diabetes during pregnancy.
  • It compares two groups of patients: those with HbA1c levels ≤ 7.0% and those with levels > 7.0%, noting significant differences in age at diagnosis, insulin needs, and gestational age at delivery.
  • Findings indicate that higher pre-pregnancy HbA1c levels correlate with increased pregnancy complications and suggest that poor glycemic control at the start of pregnancy may lead to more complicated diabetes management later on.

Article Synopsis
  • Pregnant women with type 1 diabetes (T1D) using continuous insulin infusion have higher complication risks, but pregnancy planning might help improve outcomes.
  • A study of 107 pregnancies showed that those who planned their pregnancies had better glucose control and lower risks of preterm delivery compared to unplanned pregnancies.
  • Better glucose control and reduced preterm delivery risks were linked to pregnancy planning, emphasizing the importance of preconception care for T1D patients using advanced insulin delivery methods.

Percutaneous cardiovascular interventions have changed dramatically in recent years, and the impetus given by the rapid implementation of novel techniques and devices have been mirrored by a refinement of antithrombotic strategies for secondary prevention, which have been supported by a significant burden of evidence from clinical studies. In the current manuscript, we aim to provide a comprehensive, yet pragmatic, revision of the current available evidence regarding antithrombotic strategies in the domain of percutaneous cardiovascular interventions. We revise the evidence regarding antithrombotic therapy for secondary prevention in coronary artery disease and stent implantation, the complex interrelation between antiplatelet and anticoagulant therapy in patients undergoing percutaneous coronary intervention with concomitant atrial fibrillation, and finally focus on the novel developments in the secondary prevention after structural heart disease intervention.
