Objective: To evaluate actual myocardial energy requirements using indirect calorimetry, oxygen delivery (DO), oxygen extraction ratio (OER), cardiac output (CO) and their components, as well as to study the impact of positive inotropic agent (dobutamine) on myocardial metabolism in early postoperative period after cardiac surgery.

Material And Methods: We measured the main metabolic parameters using indirect calorimetry in 20 patients after on-pump cardiac surgery with cardioplegia. To evaluate the impact of metabolic load on CO, VO and DO, we administered dobutamine 3 µg/kg/min at the second phase of the study.

Objective: To evaluate the immediate outcomes and safety of simultaneous Maze procedure in patients with isolated and multivalvular heart disease via right-sided mini-thoracotomy.

Material And Methods: A retrospective analysis of postoperative outcomes included 21 patients with various valvular heart diseases and atrial septal defects with atrial fibrillation. All patients underwent heart valve surgery with cryoablation (left atrial, right atrial or biatrial «Maze» approach) via right-sided mini-thoracotomy.

Hypoinsulinemia is a pathological consequence of diabetes mellitus that can cause a number of complications of the central and peripheral nervous system. Dysfunction of signaling cascades of insulin receptors under insulin deficiency can contribute to the development of cognitive disorders associated with impaired synaptic plasticity properties. Earlier we have shown that hypoinsulinemia causes a shift of short-term plasticity in glutamatergic hippocampal synapses from facilitation to depression and apparently involves mechanisms of glutamate release probability reduction.
