A lot of diabetic Muslims choose to fast during Ramadan, despite recommendations that advise some patients to abstain according to established risk factors. Fasting exposes the diabetic subject to acute major complications, including diabetic ketoacidosis and severe hypoglycaemia. Patients with diabetes, therefore, had to benefit from individualized care already before Ramadan, including a risk assessment, a specific education integrating in particular the adaptation of the pharmacological treatment in order to limit the occurrence of these acute complications.

This retrospective work compares the efficacy of three procedures of bariatric surgery in obese patients with type 2 diabetes : gastric bypass (n = 22), sleeve gastrectomy (n = 18) and Magenstrasse & Mill gastroplasty (also known as reversible sleeve (n = 19). The three interventions result in a remarkable weight loss after one year, a better glycaemic control, with often a remission of diabetes, a reduction of blood pressure and an improvement of lipid profile, allowing an interruption or a dose reduction of concomitant medications. Overall, classical sleeve provides almost similar results as gastric bypass and appears slightly better than reversible sleeve.

A 14-year-old girl was admitted to the Emergency department for abdominal pain. An abdominal mass of 7-8 cm was detected and surgically resected. At anatomopathology a solid pseudopapillary neoplasm of the pancreas was observed.
