Objectives: The impact of mild autonomous cortisol secretion (MACS) on cognition is incompletely characterized. We aimed to assess cognition in patients with MACS, identify factors associated with lower cognition, and determine the impact of adrenalectomy on cognition.

Methods: We conducted a cross-sectional study (4/2019 to 10/2022) and a longitudinal cohort study (10/2021 to 9/2023) of adults with MACS and referent subjects.

The 2024 European Society of Cardiology (ESC) guidelines on chronic coronary syndrome comprehensively summarize the symptoms, diagnostics and treatment of coronary artery disease, excluding acute coronary syndromes. The processing always begins with an estimation of the clinical probability, which should include not only the symptom profile but also the number of cardiovascular risk factors and is further modified by any conspicuous findings in electrocardiography (ECG), exercise testing, or echocardiography. For further diagnostics, coronary angiography via computed tomography is the first-line approach if the probability lies between 5% and 50%.

Context: Diagnosing adrenal insufficiency (AI) often requires complex testing which can be time consuming and expensive. Dehydroepiandrosterone sulfate (DHEAS) is a promising marker of hypothalamic-pituitary-adrenal (HPA) axis function, however its diagnostic performance has not yet been evaluated in a large-scale study.

Objective: Evaluate the performance of DHEAS and baseline cortisol in assessing AI.

Article Synopsis
  • This study aimed to evaluate the impact of evolocumab, a cholesterol-lowering medication, on endothelial function in patients with cardiovascular disease, using a randomized, double-blind, controlled design.
  • After 8 weeks, patients receiving evolocumab showed a significant improvement in a specific measure of endothelial function called vasoactive range (VAR) compared to those on a placebo, particularly in younger patients with lower blood pressure and higher LDL cholesterol levels.
  • Overall, the findings suggest that evolocumab helps improve endothelial function among high-risk patients already on statin therapy, which may explain its association with lower cardiovascular events observed in previous studies.

Article Synopsis
  • * The revised 2022 guidelines from the European Society of Cardiology/European Respiratory Society address PH but only briefly consider the needs of adults with congenital heart defects.
  • * The article aims to enhance understanding of ACHD management by exploring various aspects such as diagnostics, specialized therapies, and unique circumstances, thereby addressing gaps in current guidelines.
