Therapeutic endoscopic ultrasound (EUS) encompasses an array of procedures to manage pancreaticobiliary and luminal gastrointestinal disorders. Therapeutic EUS procedures include EUS-guided rendezvous of the pancreatic and biliary ducts, as well as direct drainage of the bile duct, pancreatic duct, and gallbladder, drainage of pancreatic fluid collections, and luminal anastomosis creation. These procedures have a range of required equipment, approaches, clinical outcomes, and adverse events dependent on both procedure-related and patient-related factors.

Background And Aims: Gastric variceal bleeding occurs less commonly than bleeding from esophageal varices (EVs), although it is associated with higher morbidity and mortality. Bleeding from gastroesophageal varices type 1 (GOV1) is treated like EVs. In contrast, other forms of gastric variceal bleeding, including gastroesophageal varices type 2 (GOV2) and isolated gastric varices types 1 (IGV1) and 2 (IGV2), are treated with varying endoscopic approaches.

Article Synopsis
  • A multicenter study aimed to evaluate the effectiveness of methylated DNA markers (MDMs) found in pancreatic juice (PJ) for detecting pancreatic ductal adenocarcinoma (PDAC) when used alone and in combination with a blood test, CA 19-9.
  • The analysis included DNA samples from 88 confirmed PDAC cases and 134 control cases, utilizing logistic regression to summarize accuracy through the area under the receiver-operating characteristic curve (AUROC).
  • Results showed that the combination model of PJ-MDMs and CA 19-9 achieved a higher accuracy (AUROC of 0.95) compared to using PJ-MDMs (0.87) or CA 19-9 alone (0

Background And Aims: EMR and endoscopic submucosal dissection (ESD) are minimally invasive endoscopic techniques, developed for the removal of benign and early malignant lesions throughout the GI tract. Submucosal injection of a marking agent can help to identify lesions during surgery. Endoscopic resection frequently involves "lifting" of the lesions by injection of a substance within the submucosal space to create a cushion for safe resection.

  • Atrial fibrillation (AF) ablation can lead to serious complications like esophageal injuries, which can create life-threatening fistulas; EUS (Endoscopic Ultrasound) is being explored as a better diagnostic tool than EGD (Esophagogastroduodenoscopy) for detecting these injuries.
  • A study of 234 patients showed that EUS effectively identified various conditions post-ablation, including pleural effusions and esophageal wall changes, with a strong association between certain EUS findings and the severity of esophageal injury.
  • The findings suggest that EUS is a safe and potentially superior method for evaluating post-ablation injuries in patients, with recommendations for prioritizing EUS over EGD in these

Background And Aims: Endoscopic retrograde cholangiopancreatography (ERCP) carries a 3-15% risk of post-ERCP pancreatitis (PEP). Rectal indomethacin reduces the risk of PEP, but its cost has increased more than 20-fold over the past decade. Rectal diclofenac is also used to prevent PEP but is not commercially available in the United States.

Background & Aims: There is significant variability in the immediate post-operative and long-term management of patients undergoing per-oral endoscopic myotomy (POEM), largely stemming from the lack of high-quality evidence. We aimed to establish a consensus on several important questions on the after care of post-POEM patients through a modified Delphi process.

Methods: A steering committee developed an initial questionnaire consisting of 5 domains (33 statements): post-POEM admission/discharge, indication for immediate post-POEM esophagram, peri-procedural medications and diet resumption, clinic follow-up recommendations, and post-POEM reflux surveillance and management.

Background: The authors previously developed an artificial intelligence (AI) to assist cytologists in the evaluation of digital whole-slide images (WSIs) generated from bile duct brushing specimens. The aim of this trial was to assess the efficiency and accuracy of cytologists using a novel application with this AI tool.

Methods: Consecutive bile duct brushing WSIs from indeterminate strictures were obtained.

Article Synopsis
  • This study examines how using Magnetic Resonance Cholangiopancreatography (MRCP) can help identify disconnected pancreatic duct syndrome (DPDS) in patients with walled-off necrosis (WON) to prevent the recurrence of pancreatic fluid collections (PFCs).
  • In the research, 121 patients were assessed, with those who had MRCP showing a significantly lower recurrence rate of PFCs (0%) compared to those who did not have MRCP (16.8%).
  • The findings suggest that MRCP is effective in identifying patients at risk for recurrence, enabling better management strategies and preventing subsequent PFC occurrences.

Background/aims: Coaxial placement of double pigtail plastic stents (DPPS) through lumen-apposing metal stents (LAMSs) is commonly performed to reduce the risk of LAMS obstruction, bleeding, and stent migration when used for the drainage of pancreatic fluid collections (PFCs). A systematic review and meta-analysis were performed to compare the outcomes of LAMS alone and LAMS with coaxial DPPS placement in the management of PFCs.

Methods: A systematic review was conducted to identify studies comparing LAMS and LAMS/DPPS for PFC drainage.

Background And Aims: Early identification of malignant biliary strictures (MBSs) is challenging, with up to 20% classified as indeterminants after preliminary testing and tissue sampling with endoscopic retrograde cholangiopancreatography. We aimed to evaluate the use of methylated DNA markers (MDMs) from biliary brushings to enhance MBS detection in a prospective cohort.

Approach: Candidate MDMs were evaluated for their utility in MBS diagnosis through a series of discovery and validation phases.

EDEE is a relatively safe and effective procedure when performed by expert endoscopists to establish pancreaticobiliary access in patients who have failed, or are not candidates for, traditional ERCP or alternative drainage modalities. Careful preprocedural planning with attention to the patient's specific postsurgical anatomy can optimize outcomes and minimize AEs.

One of the most effective ways to advance the performance of quantum computers and quantum sensors is to increase the number of qubits or quantum resources in the system. A major technical challenge that must be solved to realize this goal for trapped-ion systems is scaling the delivery of optical signals to many individual ions. In this paper we demonstrate an approach employing waveguides and multi-mode interferometer splitters to optically address multiple Yb ions in a surface trap by delivering all wavelengths required for full qubit control.

Background: Visceral fat represents a metabolically active entity linked to adverse metabolic sequelae of obesity. We aimed to determine if celiac artery mesenteric fat thickness can be reliably measured during endoscopic ultrasound (EUS), and if these measurements correlate with metabolic disease burden.

Methods: This was a retrospective analysis of patients who underwent celiac artery mesenteric fat measurement with endosonography (CAMEUS) measurement at a tertiary referral center, and a validation prospective trial of patients with obesity and nonalcoholic steatohepatitis who received paired EUS exams with CAMEUS measurement before and after six months of treatment with an intragastric balloon.

Background And Aims: Access to new endoscopic treatment modalities often depends on price. To resolve this gap and therefore help to ensure that care delivery can occur on a clinical basis, we aimed to establish the value to insurers of novel hemostatic powder to treat GI tumor bleeding.

Methods: A decision-analytic model developed to assess the impact of endoscopic intervention on the risk of 30-day readmission for GI bleeding from an insurer perspective was adapted to assess GI tumor bleeding with hemostatic powder or standard endoscopic therapy.
