Background: Stable Weber B ankle fractures are routinely treated nonoperatively. Our group previously presented a novel algorithm that provides radiographic parameters guiding when Weber B ankle fractures can be treated nonoperatively. The purpose of this study is to evaluate the durability of those results with a minimum 5-year follow-up.

Article Synopsis
  • Assessment of functional capacity is crucial for understanding how cognitive impairment in schizophrenia affects treatment responses, with existing methods mainly relying on paper simulations.
  • A new tool, the Virtual Reality Functional Capacity Assessment Tool (VRFCAT), was tested on 54 healthy individuals and 51 people with schizophrenia, measuring completion times and errors during a simulated shopping trip.
  • Results showed that individuals with schizophrenia performed worse than healthy controls in most measures, suggesting that computerized assessments could be beneficial, though further research is needed.

Cognitive functioning can be assessed with performance-based assessments such as neuropsychological tests and with interview-based assessments. Both assessment methods have the potential to assess whether treatments for schizophrenia improve clinically relevant aspects of cognitive impairment. However, little is known about the reliability, validity and treatment responsiveness of interview-based measures, especially in the context of clinical trials.

Cognitive impairments affect the majority of patients with schizophrenia and these impairments predict poor long term psychosocial outcomes.  Treatment studies aimed at cognitive impairment in patients with schizophrenia not only require demonstration of improvements on cognitive tests, but also evidence that any cognitive changes lead to clinically meaningful improvements.  Measures of "functional capacity" index the extent to which individuals have the potential to perform skills required for real world functioning.

mTOR inhibitors avoid calcineurin nephrotoxicity, but sirolimus de novo is associated with unacceptable side effects and higher rejection rates. We have investigated a modified strategy: alemtuzumab induction with tacrolimus and mycophenolate maintenance, switching from tacrolimus to sirolimus at 6 months and stopping mycophenolate at 12 months. Here, we report the 6-year follow-up of 30 patients prospectively recruited to this single-arm pilot study and compare outcomes to a matched contemporaneous control group of 30 patients who received standard induction and calcineurin-inhibitor-based immunosuppression.
