A 92-year-old female ex-smoker with chronic obstructive pulmonary disease (COPD) GOLD III, was admitted because of communitarian pneumonia in November 2013. She had been using inhaled corticosteroids and bronchodilators and presented five exacerbations of COPD due to pneumonia in the same year, with hospitalizations in March and September. The patient underwent the routine protocol for exacerbated COPD, and bacilloscopy for tuberculosis (TB) was negative.

The presenting manifestation was a moderate, dull and steady pain in his right scapular region, which started on the 40th postoperative day and irradiated to the back and lower limbs. The dissection and its extent was diagnosed on transthoracic echocardiography and CT scanning and the patient improved on conservative management with beta blockers.

On high-resolution computed tomography scans, the pulmonary reaction to nitrofurantoin most commonly manifests as an interstitial-alveolar pattern in both lung bases. However, in this case, the alterations were most pronounced in the periphery of the upper lobes.
