After allogeneic HSCT (allo-HSCT), the diversity of the intestinal microbiota significantly decreases. The changes can be rapid and are thought to be caused by chemotherapy, antibiotics, or intestinal inflammation. Most patients are exposed to prophylactic and therapeutic antibiotics during neutropenia and several patients are colonized by ESBL bacteria.

Introduction The anterior cruciate ligament is the most commonly injured ligament in the knee. Its injury is often evaluated with orthopedic tests during physical examination, but this turns out to be a subjective assessment. A knee arthrometer is a mechanical device developed in the 1970s to improve the diagnostic accuracy of anterior cruciate ligament injury.

Patients with facial trauma are complex; difficult airway management is often anticipated and challenging for the anaesthetist. Awake tracheal intubation is the gold standard in the management of predicted difficult airway because of its high success rate and safety profile. We present the case of a patient with facial trauma who underwent orotracheal intubation with combined videolaryngoscopy and flexible bronchoscopy, under conscious sedation with intermittent boluses of ketamine and dexmedetomidine.

New frontiers about retinal cell transplantation for retinal degenerative diseases start from the idea that acting on stem cells can help regenerate retinal layers and establish new synapses among retinal cells. Deficiency or alterations of synaptic input and neurotrophic factors result in trans-neuronal degeneration of the inner retinal cells. Thus, the disruption of photoreceptors takes place.

Antihypertensive pharmacological therapy is often characterized by a coadministration of different classes of drugs. Therefore, analytical methods allowing the simultaneous quantification of many drugs are needed for therapeutic drug monitoring (TDM) purposes in this context. In particular, TDM represents a useful tool to discriminate poor adherence from real cases of resistant hypertension.

Background/objectives: Chronic kidney disease (CKD) and end-stage kidney disease (ESKD) are significant public health issues, with cardiovascular morbidity and mortality being the leading causes of death in hemodialysis patients. Osteopontin (OPN), a multifunctional glycoprotein, has emerged as a potential biomarker for vascular disease in CKD due to its role in inflammation, tissue remodeling, and calcification.

Methods: This cohort study included 1124 hemodialysis patients from the PROGREDIRE study, a registry involving 35 dialysis units in Southern Italy.

Background: Whether extended lymphadenectomy is associated with improved disease-free and overall survival, as compared with standard lymphadenectomy, among patients with localized muscle-invasive bladder cancer undergoing radical cystectomy is unclear.

Methods: We randomly assigned, in a 1:1 ratio, patients with localized muscle-invasive bladder cancer of clinical stage T2 (confined to muscle) to T4a (invading adjacent organs) with two or fewer positive nodes (N0, N1, or N2) to undergo bilateral standard lymphadenectomy (dissection of lymph nodes on both sides of the pelvis) or extended lymphadenectomy involving removal of common iliac, presciatic, and presacral nodes. Randomization was performed during surgery and stratified according to the receipt and type of neoadjuvant chemotherapy, tumor stage (T2 vs.

Article Synopsis
  • Neural stem cells (NSCs) in adult vertebrate brains, particularly in zebrafish, can generate neurons throughout life by making balanced decisions about their fate, including direct conversions without dividing.
  • Researchers reanalyzed imaging data and found that both NSCs and neuronal progenitors share similar delamination dynamics, indicating a direct connection in how these cells transition to neurons.
  • They discovered that non-apoptotic caspase activation (Caspase3/7) in NSCs, influenced by the transcription factor Atf3, plays a significant role in determining the fate of these stem cells, especially during injury, leading to increased neuron formation and maintaining NSC population stability.

Numerous drugs are known to alter the colour of human body fluids. Although drug-induced bronchial secretions staining is normally harmless, it may frighten the patient and could lead to unnecessary clinical inquiries. Cefiderocol is often removed renally as an unmodified drug; bronchial secretion staining has not been seen at doses used in clinical practice.

Time to positivity (TTP) refers to the duration required for a microbiological culture test to indicate a positive result, marking the onset of detectable bacterial or fungal growth in the sample. Numerous variables, including patient characteristics, infection source, former antimicrobial therapy, blood sample volume, and sample transportation time can influence the value of TTP. Several studies have been conducted on bloodstream infections, whereas studies on the clinical significance of yeast TTP are quite limited in the literature.

Article Synopsis
  • This paper investigates how uncertainty quantification (UQ) can help assess the reliability of deep learning tools for segmenting white matter lesions in MRI scans of multiple sclerosis patients.
  • It focuses on two main areas: ensuring that higher uncertainty values correctly indicate potentially incorrect predictions, and examining how uncertainty can vary across different anatomical levels—like voxels, lesions, and patients.
  • The authors present new methods for measuring uncertainty at the lesion and patient scales, and their findings show that these methods are more effective at identifying model errors compared to traditional voxel-scale uncertainty measures.

Herein, we introduce an unprecedented cascade reaction for the assembly of pyrrole-fused 1,5-benzodiazepine frameworks. These diverse privileged scaffolds were controllably constructed by intramolecular cyclization of β-enamino diketone, followed by annulation with -phenylenediamine. The protocol features efficient one-pot cascade cyclization/annulation, performed under simple and mild reaction conditions.

Article Synopsis
  • The study investigates how preoperative glucose metabolism impacts the risk of clinically relevant pancreatic fistulas (CR-POPF) after pancreaticoduodenectomy (PD) surgery.
  • Out of 110 patients, those with normal glucose tolerance showed a higher incidence of CR-POPF at 51.7%, compared to those with impaired glucose tolerance or diabetes.
  • Fasting glucose levels and c-peptide values were found to be effective predictors for CR-POPF, indicating that both normoglycemia and normal beta cell function may increase the risk of complications post-surgery.
