Article Synopsis
  • * This study used cardiac magnetic resonance (CMR) imaging to evaluate myocardial health and found that increased interstitial fibrosis (measured by extracellular volume fraction, ECV%) correlates with worse health outcomes in these patients.
  • * Over a median follow-up of 5.7 years, higher ECV% was linked to increased mortality and heart failure admissions, indicating that ECV% could be a valuable tool for identifying patients needing closer monitoring.

Aims: Transthoracic echocardiography is recommended in all patients with acute coronary syndrome but is time-consuming and lacks an evidence base. We aimed to assess the feasibility, diagnostic accuracy, and time efficiency of hand-held echocardiography in patients with acute coronary syndrome and describe the impact of echocardiography on clinical management in this setting.

Methods And Results: Patients with acute coronary syndrome underwent both hand-held and transthoracic echocardiographies with agreement between key imaging parameters assessed using kappa statistics.

A key focus of cardiovascular medicine is the detection, treatment, and prevention of disease, with a move towards more personalized and patient-centred treatments. To achieve this goal, novel imaging approaches that allow for early and accurate detection of disease and risk stratification are needed. At present, the diagnosis, monitoring, and prognostication of thrombotic cardiovascular diseases are based on imaging techniques that measure changes in structural anatomy and biological function.

Background: Aortic mural thrombus is a rare acute aortic syndrome that can present with embolism to a distal organ. No guidelines or randomized evidence exist to guide therapy for patients with aortic mural thrombus. Cardiac and cerebral embolism is a particularly unusual presentation of aortic thrombus but has significant implications for patient management.

Urbanization has been considered a driver of PM pollution and the attributed health burden. This study systematically measured the spatiotemporal and urban-rural heterogeneity of PM-attributed health burden drivers, including income, population, baseline mortality rate, and PM level. The results reveal the significantly positive contribution of disposable income and the periodical and urban-rural differentiation of population contribution to PM-attributed health burden.

Article Synopsis
  • Coronary F-sodium fluoride (F-fluoride) uptake indicates activity and progression of atherosclerotic disease, but its changes over time remain unclear, especially in patients with unstable coronary artery disease.
  • A study followed 111 patients (59 with chronic coronary artery disease and 52 post-myocardial infarction) over 12 months using PET and CT angiography to measure coronary microcalcification activity (CMA) and calcification scores.
  • Results showed that while coronary F-fluoride uptake correlated with greater disease severity and faster calcification progression, it did not show significant changes over the one-year period in either patient group.

In this EACVI clinical scientific update, we will explore the current use of multi-modality imaging in the diagnosis, risk stratification, and follow-up of patients with aortic stenosis, with a particular focus on recent developments and future directions. Echocardiography is and will likely remain the key method of diagnosis and surveillance of aortic stenosis providing detailed assessments of valve haemodynamics and the cardiac remodelling response. Computed tomography (CT) is already widely used in the planning of transcutaneous aortic valve implantation.

Myocardial fibrosis is the heart's common healing response to injury. While initially seeking to optimize the strength of diseased tissue, fibrosis can become maladaptive, producing stiff poorly functioning and pro-arrhythmic myocardium. Different patterns of fibrosis are associated with different myocardial disease states, but the presence and quantity of fibrosis largely confer adverse prognosis.

Article Synopsis
  • A study was conducted to explore the effectiveness of F-GP1 PET/CT angiography in detecting thrombus formation in coronary arteries of patients with and without acute myocardial infarction.
  • Out of 99 patients, 80% of those with myocardial infarction showed significant uptake of the F-GP1 radiotracer, suggesting the presence of thrombosis, although some false negatives were noted.
  • This noninvasive imaging technique has the potential to enhance the diagnosis and treatment strategies for myocardial infarction by clearly identifying thrombus locations within the heart.

Coronary venous spasm has never been reported during premature ventricular extrasystole ablation. We report a 20-year-old female patient who experienced a severe spasm of the great cardiac vein during radiofrequency ablation for premature ventricular contractions, which were relieved eventually by the administration of intracoronary nitroglycerine. The operation was successfully completed, leading to a long-term resolution of her palpitation symptoms.
