Hyperostotic Sphenoid Wing Meningioma with Proptosis: A Rare Case Report with Literature Review.

Background: We present a case of proptosis with headache, decrease in vision and orbital pain where patient underwent sphenoid-orbital decompression, and excision of mass, resulting in significant improvement in her vision with complete resolution of proptosis, headache and orbital pain. This study was done to diagnose and evaluate clinical outcome of hyperostotic sphenoid wing meningioma with orbital involvement after surgery.

Case: A 40 year-female presented with severe headache, right eye swelling, periorbital pain and decreasing in vision for two years, and right eye proptosis more severe for three months.

Surgical Excision of Cirsoid Aneurysm of the Scalp: Case Series and Review of the Literature.

Background: Cirsoid aneurysms, also known as arteriovenous malformations (AVMs), of the scalp are relatively rare lesions. They may be found incidentally or with symptoms such as an enlarging pulsatile mass, headache, or bleeding.

Methods: This retrospective case series comprised 10 cases of scalp AVMs that were treated with surgical excision from January 2010 to January 2020.

Cerebral Tuberculoma of Right Frontal Lobe in an Unconscious Patient.

Cerebral tuberculoma of the brain are uncommon presentation of tuberculosis (1%). A 17-year female presented to the emergency with a Glasgow Coma Score of (eye-1, verbal-1 and motor-3), 5/15 bilateral fixed pupils with laborious breathing. The radiological features were suggestive of aintracerebral abscess/ glioma.

Primary Pituitary Tubercular Abscess: A Case Report.

Primary pituitary tubercular abscess is a very rare disease. It may present clinically with visual loss, headache, seizure, hormonal abnormalities or with cranial nerve palsies. MRI is the diagnostic modality and shows a cystic-solid mass in the sellar and suprasellar region, isointense on T1 and T2W images with heterogeneous areas and ring enhancement on contrast.

Traumatic Luxation of the Eye Ball with Optic Nerve Transection Following Road Traffic Accident: Report of Two Cases and Brief Review of Literature.

Background: Traumatic luxation of the eye ball is rare with only 106 cases reported in PubMed till date. The anatomic location of the eyeball within the socket and the resilience of the globe to pressure force, attachment to extraocular muscles and optic nerve prevents luxation during trauma.

Case: Road traffic accidents (RTA) is the most common cause for these injuries and the outcome can vary from complete recovery on repositioning to visual loss due to globe perforation or optic nerve injury.

Isolated cervical anterior spinal artery aneurysm: case report.

Isolated cervical anterior spinal artery (CASA) aneurysms are extremely rare with most of them mostly associated with arteriovenous malformation. The underlying pathology is not known but some factors leading to the aneurysm are inflammatory, infection and connective tissue disorders. Trauma with formation of pseudoaneurysm has also been reported.

Traumatic avulsion and bilateral eye loss: report of two cases.

Background: The optic nerve and the globes are resistant to mild to moderate trauma and bilateral avulsion of the eyes is rare with only a few cases reported in the literature.

Case: We report two cases of traumatic bilateral eye avulsion. The first case was secondary to a bear bite and was managed successfully, although the eyes were not salvageable whereas the second case which was due to physical assault expired due to associated severe head injury.

Penetrating head injury with bilateral eye avulsion due to Himalayan bear bite.

The Himalayan black bear (Ursus thibetanus or Selenarctos thibetanus), although an omnivore, is more carnivorous than its American counterpart. It is also more aggressive towards humans and is a threatened species because of the deforestation in the Himalayas. Furthermore, poverty, encroachment of the forest, extensive deforestation, lack of education and living near the forest are factors that increase the probability of such animal injuries.

Clinical, radiological and endocrinological findings in a case of McCune-Albright syndrome.

The McCune-Albright syndrome was described as a syndrome of polyostotic fibrous dysplasia, café au lait skin pigmentation, and autonomous endocrine hyperfunction in 1937. We report a 17-year girl with early menarche and accelerated growth for the past three years. The endocrinological examination showed slight rise of growth hormone with other hormones in normal range.

Unusual cause for ventriculoperitoneal shunt failure: carcinoma breast compressing distal catheter.

Insertion of a ventriculoperitoneal (VP) shunt is one of the most common surgical procedures in any neurosurgery unit worldwide. Distal catheter obstruction outside the peritoneum is a rare cause of shunt failure. We report the first case of distal obstruction in a 70-year old female by carcinoma breast engulfing the catheter and causing kinking.

Penetrating neck injury by an arrow: a paradigm of age old assault.

Penetrating neck injuries by traditional weapons are rare entity in the modern era of sophisticated weapons. We report an unusual case of penetrating neck injury by a metallic arrow entering anterior neck in zone II and its tip coming out of neck posteriorly causing difficulty for patient to lie down as well as posing challenge to intubate. Arrows are low velocity projectile and from a close proximity they can cause penetrating trauma similar to a low powered handgun.

Cerebellopontine epidermoid presenting with trigeminal neuralgia for 10 years: a case report.

Trigeminal neuralgia, also called tic douloureux, is a common and potentially disabling pain syndrome, which affects the trigeminal or fifth cranial nerve. The precise pathophysiology of Trigeminal neuralgia remains obscure. The disorder causes extreme, sporadic, sudden burning or shock-like face pain that lasts from few seconds to minutes and can be physically and mentally incapacitating.

Giant sacrococcygeal teratoma in an adult: case report.

Congenital sacrococcygeal teratoma (SCT) is the most common germ cell tumour of infancy and childhood. Authors report a case of 18-year-old female with progressively enlarging sacral mass since birth. MRI showed a solid-cystic mass with no involvement of surrounding structures and it was excised completely by the post-anal route.

Frontal recurrence of medulloblastoma five years after excision and craniospinal irradiation.

Medulloblastomas were originally classified under gliomas of the cerebellum until Bailey and Cushing in 1925 named these tumors as medulloblastoma. At present these tumors are classified under primitive neuroectodermal tumor. Surgical excision followed by craniospinal irradiation is the treatment of choice.

Single burr hole drainage for chronic subdural haematoma.

Chronic subdural haematoma is a common neurosurgical condition and surprisingly surgical treatment ranges from twist drill craniostomy to more radical membranectomy. However, the outcome is generally favourable with appropriate therapy. This retrospective study is aimed at analyzing the result of single burr hole drainage of such haematoma under local anaesthesia over a period of fifteen years.

Spontaneous intracystic hemorrhage of the arachnoid cyst: a case report and review of literature.

Arachnoid cysts represent benign cysts that occur in the cerebrospinal axis in relation to the arachnoid membrane and do not communicate with the ventricular system. We report a case of a years right handed lady, who presented to the emergency department with the complaints of headache and vomiting for one week CT scan showed extraaxial cystic lesion in the left fronto-parietal region. On the fifth day of admission, patient had sudden onset of severe headache associated with loss of consciousness for about 3-4 minutes with neck rigidity.

Cervical spondylosis in Nepalese porters.

Neck pain with or without radiculopathy and myelopathy is a very common problem in clinical practice. The incidence is believed to be higher in subgroups carrying load on their head. It has been reported in literature that radiographic spondylosis is appreciable in 25% to 50% of population by the age of 50 years and 75% to 85% by the age of 65 years.
