The external stenting of saphenous vein grafts (SVGs) during coronary artery bypass grafting (CABG) has been proven to reduce intimal hyperplasia (IH) in animal models, paving the way for human randomized controlled trials (RCTs) to be conducted. Herein, we performed a study-level meta-analysis to assess the impact of the Venous External SupporT (VEST) device, an external stent, on the outcomes of SVGs. A systematic search was conducted to identify all RCTs comparing VEST-stented to non-stented SVGs in patients undergoing CABG.

Saphenous vein grafts (SVGs) are the most frequently used conduits in coronary artery bypass grafting (CABG), but their higher rate of occlusion compared to arterial conduits remains a concern. Previous studies have shown that SVG failure is mainly driven by intimal hyperplasia, an adaptative response to higher pressures of the arterial circulation. The VEST device (Vascular Graft Solutions, Tel Aviv, Israel), an external support designed to mitigate intimal hyperplasia in SVGs, has been tested in few clinical trials (RCTs).

Background: Postoperative atrial fibrillation (POAF) is the most frequent complication of cardiac surgery. Despite clinical and economic implications, ample variability in POAF assessment method and definition exist across studies. We performed a study-level meta-analysis to evaluate the influence of POAF assessment method and definition on its incidence and association with clinical outcomes.

Background: Detailed understanding of the association between intraoperative left atrial and left ventricular diastolic function and postoperative atrial fibrillation is lacking. In this post hoc analysis of the Posterior Left Pericardiotomy for the Prevention of Atrial Fibrillation after Cardiac Surgery (PALACS) trial, we aimed to evaluate the association of intraoperative left atrial and left ventricular diastolic function as assessed by transesophageal echocardiography (TEE) with postoperative atrial fibrillation.

Methods: PALACS patients with available intraoperative TEE data (n = 402 of 420; 95.

Article Synopsis
  • CABG benefits are linked to graft patency, but there's little research on post-surgery graft assessments and the factors leading to graft failure.
  • This study analyzed data from seven randomized clinical trials with 4,413 patients to investigate graft failure rates and their relationship to clinical outcomes, focusing on events like myocardial infarctions and additional surgeries.
  • Findings showed that graft failure occurred in 33.7% of patients and was more likely in older adults, women, and smokers, while statin use appeared protective; graft failure significantly raised the risk of future heart events.

Surgery of the aortic root is a challenging operation for which different techniques have been developed and refined over the last five decades. We present a review of surgical strategies and their most relevant modifications along with a summary of the most recent evidence on early and long-term outcomes. Additionally, we provide brief descriptions of the use of the valve-sparing technique in various clinical settings, including high-risk patients such as those with connective tissue disorders or concomitant dissection.

Article Synopsis
  • The study compared outcomes of thoracoabdominal aortic aneurysm repairs in patients with Marfan syndrome to those without, analyzing data from 1997 to 2022.
  • It included 684 patients, with Marfan patients being younger and having more complex aneurysms but similar rates of major adverse events and operative mortality compared to non-Marfan patients.
  • Long-term survival rates diverged at ten years, yet no significant differences in survival were found between the two groups when adjusting for other health factors.

Background: The prognostic importance of periprocedural myocardial infarction (pMI) and its inclusion in the composite outcomes of coronary revascularization trials are controversial. We assessed whether pMI is a surrogate for all-cause or cardiac mortality and quality of life (QoL) outcomes in coronary revascularization trials.

Methods: All randomized trials comparing percutaneous coronary intervention vs coronary artery bypass grafting (MEDLINE, EMBASE, Cochrane Library) were identified.

Coronary artery bypass grafting (CABG) is the most frequently performed cardiac surgery worldwide. The reported incidence of graft failure ranges between 10% and 50%, depending upon the type of conduit used. Thrombosis is the predominant mechanism of early graft failure, occurring in both arterial and vein grafts.

Coronary artery bypass grafting is the most commonly performed cardiac surgical procedure. Conduit selection is crucial to achieving early optimal outcomes, with graft patency being likely the main driver to long-term survival. We present a review of current evidence on the patency of arterial and venous bypass conduits and of differences in angiographic outcomes.
